
Anxiety disorders are a family of conditions characterized by excessive fear, worry, or behavioral avoidance that is disproportionate to actual threat and persists despite reassurance. Clinically, they include generalized anxiety disorder (GAD), panic disorder, social anxiety disorder (SAD), specific phobias, and agoraphobia; they may also overlap with related syndromes such as obsessive-compulsive disorder and posttraumatic stress disorder. The unifying feature is maladaptive activation of threat-detection and defensive systems, producing sustained autonomic arousal, cognitive preoccupation, and functional impairment.
At the neurobiological level, anxiety involves dysregulation of cortico-limbic circuitry. The amygdala plays a central role in threat appraisal, while the prefrontal cortex and anterior cingulate cortex contribute to top-down regulation and error monitoring. In anxiety disorders, there is often reduced inhibitory control over amygdala-driven responses, leading to heightened reactivity. Neurotransmitter systems implicated include gamma-aminobutyric acid (GABA), serotonin, norepinephrine, and in some cases dopamine pathways. GABAergic deficits may reduce inhibitory tone, while serotonergic and noradrenergic alterations can increase baseline arousal and bias attention toward threat cues. Neuroendocrine factors also contribute: dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis can elevate stress reactivity and perpetuate symptoms.
Cognitively, anxiety is maintained by interpretive and attentional biases. Many patients catastrophize bodily sensations (e.g., palpitations) and interpret them as danger, creating a positive feedback loop: anxiety increases vigilance, vigilance increases perceived threat, and perceived threat further increases anxiety. In GAD, worry is often excessive and hard to control, linked to a cognitive attempt to prevent negative outcomes. However, worry functions as an avoidance strategy—reducing distress short term while preventing disconfirmation long term.
Behaviorally, avoidance and safety behaviors maintain anxiety by preventing exposure-based learning. Avoiding triggers or using maladaptive coping (e.g., carrying emergency medication at all times, repeatedly checking reassurance) reduces anxiety temporarily but blocks habituation and extinction. Panic disorder illustrates this mechanism: fear of sensations leads to avoidance of activities that might evoke symptoms, expanding the feared context network.
Clinically, recognition requires assessing symptom duration, severity, and impairment. Diagnostic evaluation typically includes screening for medical mimics (hyperthyroidism, arrhythmias, medication effects, substance-induced anxiety) and comorbidities (depression, substance use disorder, and other anxiety conditions). Symptoms commonly encompass restlessness, fatigue, concentration difficulties, irritability, muscle tension, sleep disturbance, and autonomic signs such as sweating, tremor, or shortness of breath. Panic attacks involve abrupt surges of intense fear with somatic symptoms that peak within minutes, often followed by worry about recurrence.
Evidence-based treatment integrates psychotherapy and, when indicated, pharmacotherapy. Cognitive behavioral therapy (CBT) is first-line for several anxiety disorders. CBT targets cognitive distortions, teaches skills for emotional regulation, and uses structured exposure to reduce avoidance. Exposure-based approaches leverage the principles of extinction learning: repeated, controlled contact with feared stimuli without catastrophic outcomes decreases fear responses over time. For GAD, CBT often emphasizes cognitive restructuring, worry-time techniques, intolerance-of-uncertainty work, and problem-solving to reduce perseverative cognition.
Pharmacologic options include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) as first-line agents for many anxiety disorders. These medications modulate serotonergic and noradrenergic signaling, improving emotional regulation and reducing pathological threat appraisal. Benzodiazepines can provide rapid symptomatic relief but carry risks of sedation, tolerance, dependence, and impaired cognition; they are generally reserved for short-term bridging or specific situations under careful monitoring. For panic disorder, SSRIs/SNRIs are typically used with gradual titration to reduce initial activation. In treatment-resistant cases, augmentation strategies may be considered by specialists.
Lifestyle and supportive interventions can complement formal treatment. Regular aerobic exercise, adequate sleep, and stress management reduce baseline arousal and improve resilience. Reducing stimulants (e.g., excessive caffeine), limiting alcohol, and avoiding recreational substances can mitigate symptom exacerbation. Mindfulness-based therapies may help some patients by changing the relationship to worry and bodily sensations, though they are not substitutes for exposure-based or CBT interventions when these are indicated.
Prognosis is variable but often favorable with appropriate care. Early intervention improves functional outcomes and reduces chronicity. A key clinical goal is to differentiate anxiety disorder symptoms from medical illness and from transient stress reactions, then tailor therapy to the specific maintaining mechanisms—catastrophic misinterpretation, avoidance, attentional bias, or maladaptive worry processes. Ongoing assessment of suicide risk is also essential when comorbid depression is present.
