
Avoidant/Restrictive Food Intake Disorder (ARFID) is a DSM-5-TR eating disorder characterized by persistent failure to meet nutritional and/or energy needs due to restrictive or avoidant eating behaviors, leading to clinically significant consequences such as weight loss, nutritional deficiency, dependence on enteral feeding or oral nutritional supplements, or marked psychosocial impairment. Unlike classic anorexia nervosa, ARFID is not driven by an intense fear of gaining weight or pursuit of thinness; instead, its maintenance typically reflects one (or more) of several functional pathways: sensory sensitivity to food texture, taste, color, or smell; fear of aversive consequences (e.g., choking, vomiting, or allergic reactions); or limited interest in eating/low appetite. These mechanisms can become self-reinforcing. Sensory barriers reduce exposure and learning opportunities, while fear-based avoidance prevents corrective experiences that would otherwise recalibrate perceived threat. Over time, diet narrows, caloric intake drops, and restrictive routines may entrench, with downstream effects on growth, micronutrient status, gut function, and mental health.
Clinically, ARFID presents heterogeneously. Children may show early-onset selectivity that progresses to impaired intake, while adolescents and adults may develop restriction after a negative feeding event such as gastroenteritis or choking. Medical red flags include bradycardia, orthostatic symptoms, delayed growth, persistent fatigue, and evidence of malnutrition (e.g., anemia, electrolyte disturbances). Because restrictive eating can mimic or coexist with gastrointestinal, endocrine, neurologic, or allergic conditions, evaluation requires a medical rule-out approach. Baseline assessment commonly includes weight trajectory, growth percentile documentation (pediatrics), vital signs with orthostatic measurements, dietary recall, symptom review (reflux, dysphagia, abdominal pain), and screening for constipation or vomiting. Laboratory testing may include a complete blood count, comprehensive metabolic panel (including electrolytes), iron studies, vitamin B12/folate, vitamin D, and markers of nutritional risk; additional tests are guided by history. Food allergy assessment is considered only when there is compatible history, since broad avoidance without evidence can worsen restriction.
Pathophysiologically, undernutrition can affect multiple systems. Reduced intake leads to decreased energy availability, which can impair thermoregulation, sleep, immune function, and cognitive performance. Malnutrition may also alter gastrointestinal motility and microbiome ecology, contributing to bloating, constipation, or nausea that further reinforces fear or aversion. In fear-based ARFID, attentional bias toward bodily sensations (e.g., nausea, throat discomfort) can amplify perceived threat. Sensory-based ARFID often involves heightened interoceptive and sensory reactivity, where normal variations in texture or temperature are interpreted as intolerable. Low-interest ARFID may reflect reduced drive for rewarding food cues, sometimes overlapping with depressive disorders, autism spectrum traits, or attentional dysregulation; however, the disorder diagnosis depends on functional impairment and nutritional compromise.
Treatment is multimodal and targets the specific maintaining mechanism. For medically stable patients, the cornerstone is structured behavioral nutrition rehabilitation. Cognitive-behavioral therapy for ARFID (CBT-AR) and related exposure-based interventions are designed to increase dietary variety through gradual, planned food challenges, coping skills for anxiety or aversive sensations, and reinforcement of approach behaviors. Exposure is typically hierarchical: patients progress from tolerated foods to less preferred ones while practicing skills that reduce avoidance (e.g., paced breathing, cognitive reframing, and contingency management). Family-based approaches may be appropriate for children, emphasizing parental coaching to reduce accommodating behaviors that unintentionally maintain restriction while increasing supportive, non-coercive consistency.
Pharmacotherapy is not first-line but may be considered for comorbidities or specific symptom drivers. If significant anxiety or obsessive worry is prominent, targeted anxiolytic strategies may be used in conjunction with therapy; if depression, sleep disturbance, or trauma symptoms contribute, corresponding treatments may improve engagement and appetite. Appetite stimulants (e.g., selected agents used off-label in some settings) have been studied in related contexts, but evidence for ARFID is evolving and must be individualized, carefully weighed against risks, and paired with behavioral feeding work to avoid long-term reliance.
Medical stabilization is critical when malnutrition is severe. Indications for urgent care include syncope, severe weight loss, dehydration, electrolyte abnormalities, or inability to maintain oral intake. Refeeding must be cautious to mitigate refeeding syndrome, a potentially fatal shift in fluids and electrolytes (notably phosphate) during restoration of nutrition. Monitoring typically includes frequent electrolytes, vital signs, and clinical observation for arrhythmias, edema, and rapid weight changes.
ARFID outcomes improve when care is early, coordinated across pediatrics, psychiatry/psychology, and dietetics, and when interventions directly address fear, sensory intolerance, or low interest rather than focusing solely on “willpower.” Education for caregivers and patients is essential: restrictive eating is a treatable neurobehavioral pattern, not a character flaw.
