Diagnosis requires careful assessment to distinguish anxiety disorders from medical causes (e.g., hyperthyroidism, arrhythmias, substance-induced anxiety) and from depressive disorders or trauma-related conditions. Clinicians evaluate symptom severity, functional impairment, duration, and rule out differential diagnoses. Standardized tools such as the GAD-7 or panic-specific assessments can support measurement, but diagnosis remains clinical.

Evidence-based treatments include psychotherapy, pharmacotherapy, and lifestyle interventions. Cognitive behavioral therapy (CBT) is a first-line psychological approach, targeting maladaptive thought patterns and avoidance. CBT for anxiety often incorporates psychoeducation, cognitive restructuring, and exposure-based techniques. Exposure therapy is particularly effective for phobias and panic by facilitating habituation and fear extinction through repeated, controlled confrontation with feared cues or contexts. For GAD, CBT typically focuses on worry management, problem-solving, reduction of avoidance, and techniques to shift attention away from threat monitoring.

Pharmacological options commonly include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modulate threat-related neural circuits and reduce symptom burden over time. Buspirone may be used for GAD. Benzodiazepines can provide short-term symptom relief by enhancing GABA-A inhibitory effects, but they carry risks such as sedation, tolerance, dependence, and withdrawal; therefore, they are usually limited to brief periods or specific clinical situations. For panic disorder, some patients benefit from longer-term pharmacotherapy alongside CBT.

A comprehensive care plan also addresses sleep, exercise, caffeine and substance use, and stress management. Improved sleep hygiene and regular aerobic activity can reduce physiological arousal and improve resilience, while reducing stimulant intake can mitigate somatic anxiety symptoms. Mindfulness-based approaches may help some patients disengage from threat rumination and strengthen attentional control, though they are often adjunctive to CBT.

In summary, anxiety disorders arise from the interaction of threat-sensitive neurocircuitry, cognitive appraisal biases, and stress-related biological changes, resulting in persistent fear and functional impairment.
