Anxiety: Neurobiology, Diagnostic Criteria, and Evidence-Based Treatments for Generalized Anxiety Disorder (GAD)

Anxiety is a common, evolutionarily conserved emotional state characterized by apprehension, worry, and heightened threat-related vigilance. Clinically, anxiety becomes a disorder when it is excessive, persistent, and functionally impairing, and when it is not better explained by another mental disorder, substance/medication effects, or a medical condition. The Diagnostic and Statistical Manual of Mental Disorders (DSM-5-TR) frames several anxiety disorders, including generalized anxiety disorder (GAD), panic disorder, social anxiety disorder, and specific phobias, each defined by distinct symptom clusters and triggers. This summary focuses on the core mechanisms and clinical management principles most relevant to chronic anxiety, particularly GAD.

In GAD, the central phenomenology is excessive worry occurring more days than not for at least six months, difficult to control, and accompanied by symptoms such as restlessness, being “on edge,” fatigability, difficulty concentrating, irritability, and sleep disturbance. Physiologically, anxiety involves coordinated activation of brain threat circuitry and peripheral stress-response systems. Neurobiologically, converging evidence implicates hyperresponsiveness within the amygdala and extended threat networks, altered function of the medial prefrontal cortex and anterior cingulate cortex in threat appraisal and cognitive control, and dysfunction in hippocampal contextual processing. Together, these changes can bias interpretation toward threat salience, impair flexible regulation of worry, and maintain persistent threat predictions.

At the systems level, anxiety engages the autonomic nervous system and hypothalamic–pituitary–adrenal (HPA) axis. Acute threat triggers sympathetic arousal (e.g., increased heart rate, muscle tension, and heightened startle), while chronic worry can dysregulate cortisol dynamics and sleep architecture. Sleep disruption, in turn, can exacerbate cognitive vulnerabilities by impairing attention and executive control, creating a self-reinforcing cycle. Cognitive models emphasize intolerance of uncertainty, attentional bias to threat cues, and metacognitive beliefs that worry is necessary for coping or prevention of negative outcomes. Behavioral models further stress avoidance and safety behaviors that reduce disconfirmatory learning, thereby sustaining anxiety.

Epidemiologically, anxiety disorders are among the most prevalent mental health conditions. Risk factors include female sex, lower socioeconomic status, childhood adversity, temperamentally high negative affectivity, family history of anxiety or depression, and medical comorbidities (e.g., thyroid disease, cardiac arrhythmias) or substance use (e.g., caffeine, stimulants). Comorbidity is common: anxiety frequently co-occurs with major depressive disorder, obsessive-compulsive disorder, and trauma-related disorders, and it can also overlap with somatic symptom presentations.

Diagnosis requires a careful differential. Clinicians assess symptom duration, impairment, triggers, and the pattern of worry versus panic versus phobic avoidance. Rule-outs include hyperthyroidism, pheochromocytoma, medication side effects (e.g., corticosteroids), stimulant intoxication, and withdrawal states. Screening tools (e.g., GAD-7) can quantify severity and monitor response, but they do not replace clinical evaluation.

Treatment is multimodal and typically combines psychotherapy, pharmacotherapy, and lifestyle interventions. First-line psychotherapy for GAD includes cognitive-behavioral therapy (CBT), which targets worry via cognitive restructuring, behavioral experiments, problem-solving training, and exposure to feared concerns. Metacognitive therapy and acceptance-based approaches can also be useful by reducing the struggle against intrusive thoughts and strengthening attentional control. Relaxation training and mindfulness may reduce physiological arousal and improve emotion regulation, though they are often adjunctive to core CBT techniques.

Pharmacotherapy commonly includes selective serotonin reuptake inhibitors (SSRIs) and serotonin–norepinephrine reuptake inhibitors (SNRIs). These agents modulate serotonergic and noradrenergic signaling, improving threat appraisal, cognitive control, and stress-response regulation. Dosing typically requires a therapeutic trial period; benefits may emerge gradually over several weeks. Buspirone, a partial 5-HT1A agonist, can be considered in some cases, particularly where rapid sedation avoidance is desired. Short-term benzodiazepines may be used cautiously for acute symptom relief, but they carry risks including tolerance, dependence, cognitive impairment, and withdrawal; thus they are generally limited and monitored.

Evidence-based management also includes addressing sleep, reducing caffeine and stimulant exposure, encouraging regular physical activity, and treating comorbid depression or substance use disorders. Nutritional adequacy and structured routines can support HPA-axis stabilization and sleep regularity. For refractory cases, clinicians may consider specialist referral and, depending on the patient profile, augmentation strategies.

Prognosis is variable but often favorable with appropriate treatment and adherence. Early intervention can prevent chronicity and reduce functional decline. Relapse prevention strategies emphasize continued CBT skills practice, monitoring early warning signs, and maintaining psychosocial supports.
