Evidence-based interventions include relaxation training, diaphragmatic breathing, progressive muscle relaxation, mindfulness-based approaches to reduce cognitive fusion, and attentional training to shift from self-monitoring to task-relevant cues. Pharmacotherapy is considered when symptoms are severe or persistent; first-line options in many anxiety disorders include selective serotonin reuptake inhibitors (SSRIs) and serotonin–norepinephrine reuptake inhibitors (SNRIs), with careful attention to individual risk, side effects, and the athlete’s sport-specific demands.
Acute coping during a game involves strategies that modulate arousal and improve focus. Effective approaches include pre-performance routines, arousal regulation (breathing at a pace that matches movement rhythms), implementation intentions (“If I miss, I reset on the next possession”), and brief cognitive reframing of physiological sensations. Importantly, athletes should avoid catastrophic interpretations of normal bodily activation. Over time, consistent practice of these skills can reduce baseline anxiety and improve confidence, fostering a performance environment where arousal becomes “energizing” rather than threatening.
If anxiety symptoms include chest pain, severe shortness of breath, fainting, or persistent panic-like episodes, medical evaluation is essential to exclude cardiac, respiratory, or endocrine causes. Sleep deprivation, stimulants, and certain medications can also exacerbate anxiety-like symptoms. For ongoing impairment, referral to a clinician experienced in anxiety and sports psychology can support differential diagnosis and tailored care.
