
Anxiety disorders represent a group of related conditions marked by excessive fear, worry, and/or physiological hyperarousal that are disproportionate to actual threat and cause functional impairment. The core clinical problem is not simply “feeling stressed,” but a maladaptive threat-detection and threat-response system that can become chronically activated. While transient anxiety is a normal protective emotion, anxiety disorders involve persistent or recurrent symptoms that last for weeks to years and often generalize across settings.
Neurobiologically, anxiety disorders arise from dysregulation within cortico-limbic circuits. At the cognitive level, anxiety disorders are characterized by biased information processing and threat misinterpretation. Patients may overestimate the probability and severity of negative outcomes, underestimate coping capacity, and selectively attend to threat-relevant stimuli. This is reinforced by memory and learning processes that consolidate fear associations. In generalized anxiety disorder (GAD), worry is a central maintaining factor: repetitive verbal-cognitive problem simulation becomes rigid and difficult to disengage, which paradoxically increases anxiety and impairs executive functioning. Somatic symptoms can involve muscle tension, restlessness, insomnia, fatigue, gastrointestinal discomfort, and autonomic arousal such as tachycardia or sweating. Etiologically, anxiety disorders are multifactorial: genetic susceptibility interacts with temperament, developmental factors, and stress exposure. Environmental contributors include chronic stress, adverse childhood experiences, and medical conditions that increase autonomic reactivity (e.g., hyperthyroidism). Exposure-based techniques are central for panic disorder, phobias, and social anxiety: repeated, controlled confrontation with feared stimuli without catastrophic outcomes allows inhibitory learning and extinction of fear associations. For GAD, CBT may incorporate worry scheduling, cognitive restructuring, and tolerance of uncertainty. Mindfulness-based approaches can reduce rumination and improve emotion regulation by strengthening meta-awareness of threat thoughts.
Pharmacotherapy can be indicated when symptoms are severe, chronic, or impairing, or when psychotherapy access is limited. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used as first-line medications due to robust evidence for reducing anxiety severity and preventing relapse. Dosing typically requires gradual titration and several weeks to achieve maximal effect. For acute symptom relief, some clinicians use short-term benzodiazepines, but these carry risks of sedation, cognitive impairment, falls, and dependence; therefore, they are generally used cautiously and for limited durations.
Non-pharmacological interventions include lifestyle measures that reduce baseline arousal: regular physical activity, sleep hygiene, and moderation of caffeine and alcohol. Stress-management interventions—such as relaxation training, breathing regulation, and progressive muscle relaxation—can ameliorate physiological symptoms but are most effective when integrated with skills that address cognitive and behavioral maintenance.
Prognosis varies by disorder type, duration, comorbidity, and engagement in treatment. Early intervention improves outcomes, and relapse prevention strategies—such as continued exposure practice, cognitive reappraisal skills, and monitoring of early warning signs—are recommended. Importantly, anxiety disorders should be treated as medical conditions of brain–behavior circuitry rather than personal weakness, with care tailored to symptom cluster, developmental stage, and co-occurring disorders.
