Benzodiazepines may provide short-term relief for acute anxiety but carry risks of sedation, cognitive impairment, dependence, and withdrawal; they are generally used cautiously and for limited durations. Other approaches may include buspirone for chronic GAD, beta-blockers for performance-related symptoms in selected cases, and targeted augmentation strategies under psychiatric supervision.
Management also requires addressing maintaining factors: sleep disruption, substance use (including caffeine and alcohol), physical inactivity, and ongoing psychosocial stressors. Since anxiety disorders often co-occur with major depressive disorder, substance use disorders, and other anxiety presentations, clinicians should routinely screen for comorbidity. Long-term outcomes improve when patients engage in sustained therapy, adhere to medication when indicated, and practice behavioral experiments that disconfirm threat beliefs.
Risk management includes monitoring for suicidality in comorbid depression, evaluating medication side effects, and using structured follow-up to prevent relapse. Psychoeducation about normalizing physiological arousal, learning that anxiety is not equivalent to harm, and recognizing early relapse cues supports durable recovery.
