Substance-related paranoia requires cessation of the offending agent and treatment of withdrawal or underlying psychiatric vulnerability.

Prognosis varies by cause, duration, functional impairment, and engagement with treatment. Early intervention in psychosis, adherence to medication when indicated, and sustained psychotherapy improve outcomes. Preventive strategies include sleep stabilization, stress reduction, avoiding psychoactive substances, managing comorbid anxiety/depression, and maintaining supportive social contact.
