
Podiatry is the clinical specialty devoted to prevention, diagnosis, and treatment of disorders affecting the foot, ankle, and related structures of the lower extremity. In older adults, podiatric care is especially consequential because the foot is the primary interface between the body and the ground. Foot health influences gait mechanics, balance, shock absorption, footwear tolerance, and the ability to navigate daily environments. When pedal disease is unrecognized or undertreated, mobility declines and downstream risks rise, including falls, loss of independence, pressure injuries, infection, and accelerated functional impairment.
From a biomechanical standpoint, the foot must manage load distribution across the plantar surface while accommodating age-related changes such as reduced tissue elasticity, altered fat pad cushioning, joint stiffness, and degenerative conditions of the toes and midfoot. Pain from hallux valgus, metatarsalgia, neuropathic injury, or inflammatory arthropathies can cause compensatory gait patterns—such as shortened stance time, reduced dorsiflexion, and altered stride length—that increase instability. Additionally, diminished proprioception and reduced ankle range of motion can impair balance responses during perturbations.
A central geriatric concept is that pedal pathology rarely exists in isolation. Diabetes mellitus, peripheral arterial disease, chronic kidney disease, obesity, and neurologic disorders commonly co-occur and amplify risk. Diabetic peripheral neuropathy can reduce protective sensation, permitting repetitive trauma without awareness. This drives plantar callus formation, ulceration, and infection. Vascular insufficiency from peripheral arterial disease impairs tissue perfusion and delays healing, raising the likelihood of refractory ulcers and, in severe cases, limb-threatening complications. Podiatric assessment therefore integrates symptom review (pain, claudication, numbness), physical examination (skin integrity, pulses, capillary refill, deformity alignment), and risk stratification.
In clinical practice, common podiatric problems in older adults include nail dystrophy and onychomycosis, ingrown nails, calluses and corns, biomechanical deformities (e.g., hammertoes, pes planus, equinus contracture), plantar fasciitis, tarsal tunnel syndrome, and inflammatory arthropathies. Skin disorders such as tinea pedis can serve as entry points for bacterial infection, particularly when barriers are compromised. Pressure redistribution is a recurring therapeutic theme; insoles, custom orthoses, and appropriate footwear can reduce peak plantar pressures and protect vulnerable sites.
The management strategy is typically multimodal: conservative interventions are prioritized, with escalation when needed. Footwear prescription includes assessment of shoe fit, toe box width, sole stiffness, and the ability to accommodate orthoses. Debridement of callus and pathologic nail tissue reduces localized pressure and friction, but must be performed carefully in those with neuropathy or vascular compromise. Orthotic devices can improve alignment and gait efficiency, while physical therapy can address strength, flexibility, and balance. Pharmacologic options target underlying etiologies such as neuropathic pain or inflammatory disease, but clinicians should consider medication-related side effects that affect balance and fall risk.
For high-risk patients, preventive care is not optional. Structured foot screening—at least annually for low risk and more frequently for those with neuropathy, prior ulcers, or amputation history—can detect early skin breakdown and deformity changes. Patient education is integral: daily visual inspection, hygiene, moisturization (avoiding maceration), appropriate nail trimming guidance, and prompt reporting of redness, swelling, blisters, or changes in sensation. When ulcers arise, podiatry coordinates wound care principles: offloading, infection assessment, debridement when appropriate, moisture balance, and consideration of revascularization in the setting of ischemia.
Falls represent one of the most clinically meaningful outcomes linked to foot impairment. Painful or unstable feet can destabilize the musculoskeletal system through reflexive muscle guarding and altered loading. Sensory deficits reduce detection of uneven terrain, while deformities can impair foot clearance during swing phase. Thus, podiatry contributes to fall prevention by improving comfort, gait stability, and footwear compatibility with assistive devices such as canes or walkers.
Finally, geriatric podiatry supports holistic mobility goals. By mitigating pain, protecting skin integrity, optimizing biomechanics, and reducing ulcer and infection risk, podiatry helps older adults maintain ambulation and participation in daily life. In health systems, early referral and routine foot assessments are cost-effective investments that prevent avoidable hospitalizations and limb-threatening complications.
