
Sexual dysfunction in men refers to persistent problems that interfere with sexual activity and satisfaction. It is a clinically important category because it often coexists with cardiovascular disease, diabetes, neurologic disorders, medication side effects, sleep disturbance, and depression or anxiety. The term covers multiple domains, including erectile dysfunction (inability to achieve or maintain an erection adequate for intercourse), ejaculatory disorders (delayed ejaculation, anejaculation, premature ejaculation), and problems related to sexual arousal, orgasm quality, or discomfort during sexual activity.
Epidemiologically, erectile dysfunction is common with increasing age, but it is not an inevitable part of aging. Many cases reflect systemic vascular and endothelial dysfunction. Penile erection depends on a coordinated neurovascular cascade: parasympathetic pathways trigger nitric oxide release, which increases cyclic guanosine monophosphate (cGMP) in smooth muscle, leading to relaxation of cavernosal arteries and trabecular smooth muscle. Venous outflow is simultaneously restricted by compression of subtunical venules, helping maintain erection rigidity. When this pathway is impaired—by atherosclerosis, hypertension, smoking, obesity, dyslipidemia, hypogonadism, or neuropathy—erectile performance deteriorates.
Medication-induced sexual dysfunction is another major driver. Antidepressants, particularly selective serotonin reuptake inhibitors, may cause delayed orgasm or ejaculatory latency. Antihypertensives and some antipsychotics can also contribute through altered autonomic tone or hormonal effects. Substance use—especially opioids, heavy alcohol intake, anabolic steroids, and nicotine—can impair libido, erection quality, or ejaculation. Chronic inflammatory states, chronic kidney disease, and endocrine disorders such as low testosterone can further disrupt sexual physiology.
Ejaculatory disorders have distinct mechanisms. Premature ejaculation can involve serotonergic modulation and sensitivity of the ejaculatory reflex circuitry. Delayed ejaculation often reflects medication effects, pelvic floor dysfunction, psychological factors, or neurologic lesions. Anejaculation can be caused by retrograde ejaculation (e.g., from alpha-adrenergic blockade), failure of emission due to nerve injury, or spinal/brainstem pathology. These diagnoses require careful history and, when indicated, focused neurologic evaluation.
Psychological and interpersonal factors are also clinically relevant. Performance anxiety can create a feedback loop in which worry heightens sympathetic activation, reducing parasympathetic-mediated penile smooth muscle relaxation. Depression is associated with reduced libido and impaired arousal. Trauma-related symptoms may manifest as avoidance, discomfort, or difficulty sustaining arousal. Importantly, sexual dysfunction is often multifactorial: biological vulnerability plus stress or relationship dynamics can worsen symptoms even when initial physiology is intact.
Evaluation should be structured and evidence-based. Clinicians begin with a comprehensive sexual history: onset, duration, consistency across partners, nocturnal/morning erections, libido changes, ejaculation pattern, pain, and medication/substance exposure. Differential diagnosis hinges on whether erections fail at initiation, maintenance, or both, and whether ejaculation is delayed, absent, or premature. A general medical review should include blood pressure, body mass index, cardiovascular risk assessment, and screening for diabetes. Laboratory tests are tailored but commonly include fasting glucose or HbA1c, lipid profile, morning total testosterone (with repeat testing if low), and—when indicated—prolactin, thyroid-stimulating hormone, renal function, and liver enzymes.
Treatment is matched to the cause. First-line therapy for erectile dysfunction frequently involves phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil, vardenafil, avanafil), which enhance the cGMP pathway but require intact nitric oxide signaling. These agents are contraindicated with nitrates due to risk of profound hypotension and require caution with certain cardiovascular conditions. If oral therapy is ineffective or unsuitable, options include vacuum erection devices, intracavernosal injections (e.g., alprostadil), and in select cases intraurethral therapy. Penile implant surgery is reserved for refractory cases.
For hypogonadism, testosterone replacement may improve libido and some aspects of erectile function, but it is not a universal solution and requires monitoring for contraindications such as prostate or breast cancer and for hematocrit elevation. Ejaculatory disorders may be treated with behavioral techniques, medication adjustments, pelvic floor therapy, or pharmacologic strategies depending on subtype. When depression or anxiety is prominent, integrated psychosexual counseling and evidence-based mental health treatment can improve outcomes.
Given the overlap with systemic disease, sexual dysfunction should be viewed as a potential marker of cardiovascular risk. Early identification enables management of underlying conditions and can improve both sexual and overall health.
