
Post-traumatic stress disorder (PTSD) is a psychiatric condition that can develop after exposure to actual or threatened death, serious injury, or sexual violence. The core clinical issue is not simply fear at the time of trauma, but a persistent maladaptive pattern in which the brain and body remain in a heightened threat state long after the event has ended. Patients may report intrusive memories, nightmares, flashbacks, or intense psychological distress when exposed to reminders. They may also experience avoidance of thoughts, people, places, or conversations associated with the trauma, along with negative changes in mood and cognition such as persistent guilt, diminished interest in activities, detachment from others, or inability to experience positive emotions.
Clinically, PTSD is characterized by four symptom clusters. First, intrusion symptoms include unwanted distressing memories, recurrent nightmares, dissociative reactions (e.g., flashbacks), and psychological distress or physiologic reactivity to cues reminiscent of the trauma. Second, avoidance symptoms involve efforts to prevent exposure to trauma-related internal experiences (thoughts, feelings) and external reminders (locations, conversations, activities). Third, negative alterations in cognition and mood include amnesia for key aspects of the event, persistent negative beliefs about oneself or the world, distorted blame of self or others, persistent negative emotional state, markedly diminished interest, feelings of detachment, and inability to feel positive emotions. Fourth, alterations in arousal and reactivity may include irritable or aggressive behavior, reckless or self-destructive behavior, hypervigilance, exaggerated startle response, problems with concentration, and sleep disturbance.
Neurobiologically, PTSD reflects dysregulation across fear-learning and threat-response systems. Functional imaging and neuroendocrine findings implicate abnormal interactions among the amygdala, hippocampus, medial prefrontal cortex, and related networks. The amygdala supports threat detection and emotional salience; when hyperactive, it can amplify fear responses and drive intrusion. The hippocampus is important for contextual memory; impaired processing can lead to trauma memories becoming fragmented and cue-bound, which contributes to flashbacks and intense reactions to reminders. The medial prefrontal cortex contributes to top-down regulation and extinction of fear; reduced regulatory control can impair the ability to extinguish threat associations. Stress-axis dysregulation, including altered cortisol signaling and autonomic nervous system changes, supports the persistent “on guard” physiologic state. This threat system can be triggered by stimuli that resemble aspects of the trauma, even when the current environment is safe.
Risk factors include prior trauma exposure, severity and proximity of the traumatic event, lack of social support, pre-existing psychiatric conditions (including anxiety or depression), and factors that affect stress tolerance. Notably, PTSD is distinct from acute stress disorder; PTSD requires symptoms to persist for more than one month and causes clinically significant distress or impairment in social, occupational, or other important areas.
Evidence-based treatments are strongly supported by randomized trials and clinical guidelines. Psychotherapy is typically first-line. Trauma-focused cognitive behavioral therapy (TF-CBT) targets maladaptive appraisals and avoidance behaviors while using structured exposure to trauma-related cues. Prolonged exposure therapy helps patients reduce fear and physiological reactivity through gradual, repeated confrontation with trauma memories and safe cues, often using imaginal exposure and in vivo exposure. Eye movement desensitization and reprocessing (EMDR) facilitates processing of traumatic memories via bilateral sensory stimulation combined with recall and cognitive restructuring. For some patients, pharmacotherapy is used alone or as an adjunct. Selective serotonin reuptake inhibitors (SSRIs) such as sertraline and paroxetine have evidence for reducing core PTSD symptoms; serotonin-norepinephrine reuptake inhibitors such as venlafaxine may also be used. Prazosin can be considered for PTSD-associated nightmares, reflecting its role in modulating noradrenergic activity during sleep.
Adjunctive strategies include sleep-focused interventions, psychoeducation, and coping skills to manage hyperarousal. Integrated care addressing comorbid conditions—such as major depressive disorder, substance use disorders, panic disorder, or chronic pain—is crucial because comorbidity can worsen symptom burden and impede recovery. Clinicians should also consider safety planning and risk assessment, since PTSD is associated with increased risk of suicidal ideation in some individuals.
Prognosis varies. Early identification and timely access to trauma-focused therapy can improve outcomes. Nevertheless, untreated PTSD may become chronic, with persistent distress, interpersonal difficulties, and reduced occupational functioning. Mechanistically, symptom maintenance can be driven by avoidance (which prevents disconfirming experiences), persistent negative beliefs, and conditioned fear responses to trauma reminders.
If PTSD symptoms appear after a traumatic event—especially intrusive memories, nightmares, avoidance, and hypervigilance—professional evaluation is recommended. Assessment includes confirmation of trauma exposure, symptom duration, and functional impairment, as well as screening for medical contributors and other mental disorders. With appropriate, evidence-based treatment, many patients experience meaningful symptom reduction and improved quality of life.
