
Meralgia paresthetica is a neuropathic pain syndrome caused by compression or entrapment of the lateral femoral cutaneous nerve (LFCN), a purely sensory nerve that supplies sensation to the anterolateral thigh. The condition is clinically important because it can mimic musculoskeletal or hip disorders while requiring a distinct diagnostic approach focused on nerve distribution, provocation maneuvers, and risk-factor modification. Although meralgia paresthetica is often self-limited, persistent symptoms may reflect ongoing nerve irritation and can lead to chronic neuropathic pain.
Pathophysiology centers on mechanical compression of the LFCN as it passes through or near the inguinal region and the fascia lata. The LFCN emerges from the lumbar plexus (primarily L2–L3) and courses toward the anterior thigh, where it becomes susceptible to entrapment under conditions that increase pelvic or inguinal pressure or reduce local space. Common contributors include tight belts, obesity, rapid weight gain, pregnancy, postoperative scarring, repetitive hip flexion, prolonged standing, and direct trauma. Secondary etiologies include diabetes-associated microvascular vulnerability of peripheral nerves and anatomical variants that predispose to entrapment.
Patients typically report unilateral burning, tingling, numbness, or “pins and needles” localized to the anterolateral thigh. Classic features include sensory symptoms without true motor weakness, absence of reflex changes, and discomfort that worsens with hip extension, standing, or tight clothing and improves with hip flexion or positional relief. Because the LFCN is sensory, meralgia paresthetica does not cause weakness of hip flexion, knee extension, or gait in the way radiculopathy or femoral nerve lesions can. On examination, light touch or pinprick can reveal decreased sensation in the affected dermatomal area, while motor testing remains normal.
Differential diagnosis is essential. LFCN entrapment must be distinguished from lumbar radiculopathy (L2–L3 or L4 involvement), diabetic neuropathy, sacroiliac disorders, hip osteoarthritis, trochanteric bursitis, and peripheral nerve lesions such as femoral neuropathy. Red flags for alternative diagnoses include progressive neurologic deficits, back pain with leg weakness, sensory loss extending beyond the anterolateral thigh, bowel or bladder symptoms, or systemic features such as fever or unexplained weight loss.
Diagnosis is often clinical, supported by symptom mapping to the LFCN distribution and reproduction of symptoms with aggravating positions. In uncertain cases, electrodiagnostic studies may be considered, though they can have limited sensitivity because the nerve is small and purely sensory. Ultrasound can sometimes visualize nerve abnormalities and guide diagnostic/therapeutic injections. Magnetic resonance imaging of the lumbar spine or pelvis is reserved for suspected alternative pathology or when symptoms do not conform to the expected pattern.
Initial management prioritizes reducing mechanical compression and addressing modifiable risk factors. Patients are advised to loosen belts or tight garments, avoid prolonged standing, and consider weight reduction when relevant. Activity modification—such as reducing repetitive hip flexion and optimizing posture—may decrease intrapelvic and inguinal pressure. Physical therapy can incorporate hip mobility, trunk stabilization, and stretching strategies that improve space around the nerve and reduce compensatory muscle tension.
Pharmacologic therapy targets neuropathic pain pathways. Options may include topical agents (e.g., lidocaine), oral neuropathic pain medications such as gabapentinoids (gabapentin or pregabalin) or serotonin-norepinephrine reuptake inhibitors (e.g., duloxetine), and short courses of analgesics as needed. The choice depends on comorbidities, tolerability, and contraindications. If conservative measures fail, image-guided nerve blocks using local anesthetic with or without corticosteroid can provide diagnostic confirmation and therapeutic relief by reducing local inflammation and interrupting pain signaling.
Surgical intervention is considered for refractory cases lasting months despite comprehensive conservative therapy. Procedures may include decompression of the LFCN at the site of entrapment. Outcomes vary, but timely treatment before long-standing nerve damage improves the likelihood of symptom resolution. Even after successful interventions, some patients may experience residual numbness, reflecting axonal injury and sensory reorganization.
Prognosis is generally favorable when compression is relieved early. Many patients improve with conservative management, particularly when the underlying mechanical driver—such as tight clothing, obesity, or pregnancy-related pressure—is addressed. However, chronic symptoms can emerge if the nerve remains irritated over time, leading to sensitization of peripheral and central pain pathways.
Education and self-management are key: patients should track symptom triggers, avoid tight inguinal pressure, and seek care if symptoms broaden beyond the typical sensory territory or if neurologic deficits develop. A structured evaluation ensures appropriate differentiation from radiculopathy and other neurologic conditions, enabling targeted treatment rather than prolonged empiric musculoskeletal care.
