
Campylobacteriosis is an acute infectious enteritis caused primarily by Campylobacter jejuni and, less commonly, Campylobacter coli. Clinically, it manifests as diarrhea that may be watery or bloody, abdominal cramping, fever, malaise, and sometimes nausea and vomiting. The incubation period is typically 2 to 5 days, though it can range from about 1 to 10 days depending on bacterial inoculum and host factors. Because symptoms can mimic other causes of infectious diarrhea, accurate clinical assessment and, when indicated, microbiologic testing are essential.
Transmission occurs through ingestion of contaminated food or water, direct contact with infected animals (especially poultry and livestock), or cross-contamination in food preparation. A key biological feature is the organism’s microaerophilic growth requirements, which correspond to its survival in the gastrointestinal tract. On entry to the human gut, Campylobacter adheres to intestinal mucosa and invades or damages epithelial cells, triggering an inflammatory response. Virulence factors include motility mechanisms that support mucosal penetration, as well as toxins and secretion system products that contribute to epithelial injury and local immune activation.
Risk is increased by undercooked poultry, unpasteurized milk, untreated water, and inadequate hand hygiene after handling raw meat or after contact with animals. Immunocompromised individuals, young children, older adults, and pregnant patients are more likely to experience severe disease or complications. Environmental exposure is also relevant in outbreak settings, where contaminated water sources or food supply chains can accelerate transmission.
Most cases are self-limited and resolve within about a week; however, the inflammatory burden can be substantial. Dehydration from fluid losses is a major immediate concern and may lead to hypotension, tachycardia, electrolyte disturbances, and acute kidney injury in vulnerable patients. A particularly important post-infectious complication is Guillain-Barré syndrome (GBS), an immune-mediated demyelinating neuropathy that can occur after Campylobacter infection. The pathophysiology involves molecular mimicry: immune responses against bacterial antigens may cross-react with peripheral nerve components, producing ascending weakness, paresthesias, and autonomic dysfunction.
Diagnosis is guided by the clinical syndrome and epidemiologic context. Stool testing is recommended for severe illness, dysentery, prolonged symptoms, or high-risk patients. Nucleic acid amplification tests and stool culture remain important, with antimicrobial susceptibility testing increasingly relevant due to regional resistance patterns. Routine laboratory findings may include leukocytosis and elevated inflammatory markers; however, these are nonspecific and should not replace microbiologic confirmation when clinically needed.
Management begins with supportive care. Oral rehydration solution is first-line for mild to moderate dehydration; intravenous fluids are indicated when oral intake is insufficient or shock is present. Antimotility agents may be avoided or used cautiously in suspected invasive bacterial diarrhea because they can prolong infection or worsen toxicity. Nutrition should continue as tolerated.
Antibiotic therapy can shorten symptom duration in selected cases, particularly severe disease characterized by high fever, marked abdominal tenderness, or bloody diarrhea, and in high-risk hosts. Evidence-based guidelines generally favor macrolides (for example, azithromycin) in many regions, while fluoroquinolone use may be limited by increasing resistance. Treatment decisions should incorporate local resistance data and individual contraindications. Importantly, antibiotic stewardship matters: unnecessary antibiotics may not benefit mild cases and can increase adverse events and resistance selection.
Complication monitoring includes watching for dehydration, persistent or worsening abdominal symptoms suggesting complications such as ileus or invasive disease, and neurologic symptoms that could indicate GBS. If neurologic deficits arise—such as progressive weakness, difficulty walking, facial weakness, or respiratory involvement—urgent evaluation is required. GBS management may involve hospital admission, neurologic monitoring, and disease-modifying therapy such as intravenous immunoglobulin or plasmapheresis, depending on severity and timing.
Prevention focuses on food safety and hygiene: thoroughly cooking poultry to safe internal temperatures, avoiding cross-contamination between raw meat and ready-to-eat foods, washing hands with soap after handling raw products, and avoiding unpasteurized dairy. For water safety, appropriate treatment or boiling reduces risk. In settings with outbreaks, public health interventions target suspected food sources and reinforce sanitation.
In summary, campylobacteriosis is a common cause of acute infectious diarrhea with characteristic incubation, inflammatory gastrointestinal pathology, and potential immune-mediated sequelae such as Guillain-Barré syndrome.
