Social anxiety disorder involves fear of scrutiny and negative evaluation in social or performance situations. Individuals may avoid events, endure them with marked distress, or perform with significant anxiety. The condition is maintained by fear of embarrassment, safety behaviors (e.g., hiding tremor, rehearsing), and post-event rumination. These behaviors reduce corrective learning in therapy by preventing disconfirmation of feared outcomes.
Specific phobias are circumscribed fears linked to particular objects or situations, such as heights or animals. They are maintained by avoidance and heightened salience of the feared stimulus. Exposure-based interventions are therefore central.
Agoraphobia refers to anxiety about situations where escape might be difficult or help unavailable during unexpected panic or panic-like symptoms. This may develop following panic disorder, with avoidance expanding over time. Treatment often targets both panic mechanisms and mobility/avoidance patterns.
Diagnosis across anxiety disorders relies on symptom duration, intensity, associated impairment, and exclusion of alternative causes. Clinicians consider medical differentials (thyroid disease, arrhythmias, respiratory disorders), substance/medication effects (stimulants), and comorbidities such as major depressive disorder, obsessive-compulsive disorder, and posttraumatic stress disorder. The presence of excessive anxiety and worry may be difficult to distinguish from normative stress responses; however, anxiety disorders involve persistent, functionally impairing patterns and may include physiologic symptoms.
Treatment is evidence-based and typically multimodal. For GAD, CBT often includes worry management strategies, problem-solving training, and mindfulness-based approaches that reduce reactivity to intrusive thoughts.
Pharmacotherapy can be used for moderate to severe symptoms, significant impairment, or when rapid symptom reduction is needed. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used because they modulate fear and worry-related neural networks over time. Benzodiazepines may provide short-term relief for acute anxiety but carry risks including sedation, dependence, cognitive impairment, and reduced effectiveness as stand-alone long-term therapy. Buspirone is sometimes used for GAD. Beta-blockers can reduce peripheral symptoms such as tremor in performance situations, though they do not directly treat cognitive fear.
Lifestyle and adjunctive measures support recovery: regular sleep, aerobic exercise, caffeine reduction, and structured stress management can reduce baseline arousal. Psychoeducation helps patients understand the threat-detection versus danger-mapping mismatch that drives symptoms.
Prognosis varies by disorder and comorbidity but is generally improved with early recognition, consistent psychotherapy, and appropriate medication management when indicated. Relapse prevention strategies include maintaining exposure practices, continuing skills training, and monitoring triggers. Ultimately, anxiety disorders are treatable neurobehavioral conditions where modifying cognitive interpretations and reducing avoidance can recalibrate threat circuits and restore functional wellbeing.
