
Anxiety disorders represent a group of psychiatric conditions characterized by excessive fear, worry, and behavioral or cognitive symptoms that are disproportionate to actual threat and persist over time. Clinically, they are not merely “feeling stressed” but involve dysregulated threat processing across brain circuits, altered autonomic reactivity, and maladaptive learning that amplifies perceived danger.
At the core of many anxiety disorders is the amygdala–prefrontal cortex–hippocampal network. The amygdala detects potential threats and can become hyperresponsive, while the medial prefrontal cortex and anterior cingulate cortex normally help regulate emotional responses and inhibit catastrophic interpretations. In anxiety disorders, top-down control can be weakened, promoting persistent worry and heightened vigilance. The hippocampus contributes contextual memory; overgeneralization of fear to safe or only mildly related cues is common, especially when anxiety begins in adolescence or follows trauma.
Neurotransmitter and neuroendocrine factors also shape symptom emergence. Dysregulation of serotonin, norepinephrine, and gamma-aminobutyric acid (GABA) signaling can increase arousal and impair inhibitory pathways. Stress-axis activation involves corticotropin-releasing factor and hypothalamic–pituitary–adrenal (HPA) axis changes; chronic or repeated stress may sustain elevated cortisol responses and sensitization of threat circuitry. Additionally, interoceptive sensitivity—how strongly bodily sensations are perceived and interpreted—plays a significant role. When physical signals (e.g., palpitations, gastrointestinal tension, dyspnea) are interpreted catastrophically, anxiety escalates further, creating a positive feedback loop.
Several diagnoses fall under the anxiety disorders umbrella. Generalized anxiety disorder (GAD) involves excessive worry occurring more days than not for at least several months, with difficulty controlling worry and associated symptoms such as restlessness, fatigue, impaired concentration, irritability, muscle tension, and sleep disturbance. Panic disorder is defined by recurrent unexpected panic attacks followed by persistent concern about additional attacks or maladaptive behavioral change. Specific phobias center on circumscribed feared stimuli, while social anxiety disorder involves fear of scrutiny and negative evaluation. Agoraphobia includes fear of situations where escape might be difficult or help unavailable.
Psychological mechanisms include intolerance of uncertainty, attentional bias toward threat, and repetitive safety behaviors that reduce anxiety short-term but maintain fear long-term. Cognitive distortions such as overestimation of danger and underestimation of coping ability can become entrenched. Learning models emphasize that avoidant behavior reduces exposure to corrective experiences, reinforcing anxiety. Trauma-related conditions are closely related in mechanisms but are classified separately; nonetheless, heightened startle and hyperarousal may overlap with anxiety syndromes.
In assessment, clinicians establish symptom type, duration, severity, and functional impairment, and they evaluate differential diagnoses. Medical and substance-related contributors must be considered, including hyperthyroidism, arrhythmias, medication effects (e.g., stimulants), caffeine excess, and withdrawal states. Comorbidities are common: depression, other anxiety disorders, obsessive-compulsive disorder, posttraumatic stress disorder, and substance use disorders can coexist and influence treatment planning.
Treatment is multimodal and typically evidence-based. First-line psychotherapy includes cognitive behavioral therapy (CBT), which targets maladaptive worry processes, threat interpretations, and avoidance. For GAD, CBT often incorporates cognitive restructuring, worry exposure, and skills training for emotion regulation. For panic disorder and phobias, exposure-based approaches are central, using gradual, planned contact with feared sensations or situations to extinguish fear and rebuild confidence. Acceptance-based strategies may help when control is not feasible and may reduce experiential avoidance.
Pharmacotherapy depends on diagnosis, symptom severity, comorbidity, and patient preferences. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used for GAD, social anxiety disorder, and panic disorder. Dose titration and adherence are important because therapeutic benefits may require several weeks. Short-term benzodiazepines can be considered for acute symptom relief in select cases, but risks include sedation, cognitive impairment, dependence, and withdrawal; therefore, they are generally limited in duration and carefully monitored.
When anxiety is severe or treatment-resistant, augmentation strategies may be considered under specialist care, including careful trials of alternative or additional pharmacologic agents and enhanced psychotherapy. Lifestyle and supportive measures—regular sleep, structured physical activity, reduced stimulant and alcohol use, and stress management—can meaningfully improve baseline vulnerability, though they typically complement rather than replace targeted treatment.
Education and early intervention improve outcomes. Effective therapy reduces avoidance, recalibrates threat learning, and strengthens prefrontal regulation of fear responses. Family and workplace accommodations may help prevent functional decline during treatment initiation. Importantly, persistent anxiety warrants professional evaluation rather than solely relying on coping tactics, because untreated anxiety can lead to chronic disability, increased healthcare utilization, and higher risk for comorbid depression.
