In GAD, worry is typically pervasive across multiple life domains (work, health, finances, social roles), with a chronic “mental engine” that generates uncertainty-driven scenarios. Clinically, GAD requires that worry be difficult to control and associated with symptoms such as restlessness, fatigue, poor concentration, irritability, and sleep disturbance. For panic disorder, anxiety presents episodically as sudden surges of intense fear or discomfort accompanied by somatic panic symptoms (e.g., dyspnea, chest pain, dizziness, paresthesias). Social anxiety disorder centers on fear of negative evaluation, leading to avoidance or marked distress during social or performance situations. Because anxiety disorders overlap with medical conditions, careful differential diagnosis is essential. Thyroid dysfunction (e.g., hyperthyroidism), arrhythmias, pheochromocytoma, chronic obstructive pulmonary disease, substance or medication effects (e.g., stimulants, caffeine excess, withdrawal from benzodiazepines or alcohol), and neurological conditions can mimic anxiety presentations. Mood disorders also require differentiation: major depressive disorder may coexist but has distinct depressive symptoms; bipolar disorder requires screening for past hypomanic or manic episodes. Posttraumatic stress disorder (PTSD) is distinguished by trauma exposure and intrusive trauma symptoms rather than generalized fear.

Evidence-based treatment is multimodal and should be tailored to symptom profile, comorbidities, and patient preference. First-line psychotherapy for many anxiety disorders is cognitive-behavioral therapy (CBT), which targets maladaptive beliefs, attentional bias, and avoidance behaviors. Exposure-based strategies are central: graded and systematic exposure helps extinguish conditioned fear responses and improves inhibitory learning. For GAD, CBT often includes worry exposure, intolerance-of-uncertainty interventions, and cognitive restructuring. Mindfulness-based approaches can reduce rumination and improve meta-cognitive control, though CBT remains the most consistently supported.

Pharmacotherapy commonly uses selective serotonin reuptake inhibitors (SSRIs) or serotonin-norepinephrine reuptake inhibitors (SNRIs) due to their favorable long-term benefit-risk profile and ability to modulate threat-related neural networks. Dose titration and adequate trial duration are important because early improvement may be gradual. For panic disorder and severe acute anxiety, short-term benzodiazepines may be considered in select cases, but risks include sedation, falls, tolerance, dependence, and cognitive effects; thus, they are typically time-limited and paired with psychotherapy when appropriate.

Lifestyle and adjunctive measures can support treatment response. Regular aerobic exercise improves autonomic balance and may reduce baseline anxiety. Sleep optimization is crucial because insomnia and circadian disruption can increase emotional reactivity. Reducing stimulants (excess caffeine, nicotine) and addressing substance use lowers physiological arousal that can amplify anxiety symptoms.

Finally, prognosis varies by disorder type, severity, and comorbidity, but many patients experience substantial improvement with appropriate, sustained therapy. Clinicians should screen for comorbid depression, substance misuse, and medical mimics, and provide a collaborative care plan emphasizing skills acquisition, exposure practice, and medication adherence when indicated.
