Risks and cautions include ensuring adequate protein and overall energy intake, particularly in older adults or those with high activity needs. Individuals with specific medical conditions—such as chronic kidney disease requiring protein restriction, or diabetes requiring glucose-lowering medication adjustments—should personalize dietary changes with clinicians or registered dietitians. Additionally, “healthy” does not mean unlimited: calorie density of nuts and olive oil can be beneficial but still adds up.
Overall, the Mediterranean diet provides a biologically plausible, evidence-informed strategy to support healthy weight loss and cardiometabolic risk reduction.
