Home health services, outpatient therapy, or inpatient rehabilitation may be recommended depending on severity and endurance. A key question in discharge is risk forecasting: fall risk, choking risk, ability to attend therapy sessions, and likelihood of rehospitalization due to complications.

Medical complications must be anticipated throughout rehab. Post-stroke spasticity may require stretching regimens, orthoses, or pharmacologic options such as botulinum toxin injections for focal spasticity. Neuropathic pain can require multimodal management (e.g., neuropathic agents and behavioral strategies). Cardiovascular monitoring is essential because stroke survivors remain at elevated risk for recurrent ischemic events. If atrial fibrillation is present, anticoagulation decisions must balance thromboembolic risk against bleeding risk, with adherence and monitoring emphasized during transitions of care.

Timing is clinically sensitive. While many functional gains occur in the first months, improvements can continue beyond that window with appropriate therapy. Therefore, “discharge timeline” should be framed as a dynamic clinical milestone rather than a fixed date.
