
Erectile dysfunction (ED) is the persistent or recurrent inability to achieve and/or maintain an erection sufficient for satisfactory sexual performance. Clinically, ED is not merely a psychosexual issue; it reflects impaired vascular, neurologic, hormonal, or psychological function and often serves as an early marker of systemic disease. Prevalence increases with age, but ED can occur in younger men as well, especially when risk factors such as metabolic syndrome, medication exposure, sleep disorders, or substance use are present.
Physiology of normal erection involves coordinated arteriovenous blood flow, intact peripheral and central nerves, and appropriate hormonal signaling. Sexual stimulation triggers parasympathetic-mediated release of nitric oxide in penile tissue, activating guanylate cyclase and increasing cyclic GMP, which relaxes smooth muscle within the corpora cavernosa. This promotes arterial inflow, veno-occlusion, and sustained erection. ED may result from diminished nitric oxide bioavailability, insufficient smooth muscle relaxation, impaired endothelial function, inadequate neural signaling, or failure of the veno-occlusive mechanism.
Atherosclerotic and endothelial dysfunction are common drivers. Because penile arteries are small, ED may manifest earlier than overt coronary artery disease. Diabetes mellitus, chronic hypertension, dyslipidemia, smoking, and obesity accelerate endothelial injury and reduce nitric oxide signaling. Neurologic causes include spinal cord injury, multiple sclerosis, peripheral neuropathy (often in diabetes), and post-surgical nerve disruption (e.g., after radical prostatectomy). Hormonal etiologies include hypogonadism, where low testosterone can reduce libido and impair erectile capacity, though it is not the sole cause in most patients.
Medication-induced ED is frequent: antihypertensives (especially nonselective beta-blockers and some thiazides), antidepressants (notably SSRIs), antipsychotics, finasteride, and certain opioids can contribute. Lifestyle factors such as heavy alcohol use, illicit drug use (e.g., cocaine), sedentary behavior, and untreated sleep apnea are also implicated. Sleep apnea can worsen sympathetic tone and endothelial function, further impairing erections.
Psychological factors can be both primary and secondary. Performance anxiety, maladaptive beliefs about sexual competence, depression, and relationship stress can create a vicious cycle: anxiety heightens sympathetic activation, which counteracts parasympathetic erection pathways, leading to failure and increased fear of recurrence. Even when an organic cause exists, anxiety can amplify symptom severity.
Evaluation begins with confirming the pattern and duration of symptoms, assessing nocturnal or morning erections, and reviewing medications, comorbidities, and lifestyle. Clinicians typically screen for cardiovascular risk because ED correlates with systemic vascular health. Recommended baseline labs often include fasting glucose or HbA1c for diabetes, a lipid panel, and morning total testosterone. Additional testing may include thyroid function (for suspected hypothyroidism), prolactin (if indicated for hyperprolactinemia symptoms), and kidney/liver function depending on clinical context. If neurologic symptoms, significant trauma, or complex etiologies are present, referral to urology and possible specialized testing may be warranted.
First-line treatment frequently involves addressing modifiable risks: smoking cessation, weight reduction, regular aerobic and resistance exercise, improved glycemic control, and limiting alcohol or substance use. Psychosexual interventions can be essential when anxiety or relationship dynamics contribute. Evidence-based behavioral strategies include cognitive-behavioral therapy, performance-focused counseling, and addressing communication and expectations with the partner.
Pharmacologic therapy is often central. Phosphodiesterase type 5 inhibitors (PDE5 inhibitors) improve erectile response to sexual stimulation by preserving cyclic GMP in penile tissue. Common agents include sildenafil, tadalafil, and others in the class. These drugs are contraindicated with nitrates due to risk of severe hypotension. Patients should be evaluated for cardiovascular stability before sexual activity, especially if they have significant cardiac disease.
For individuals who cannot use or do not respond to PDE5 inhibitors, options include vacuum erection devices, intracavernosal injections (e.g., alprostadil or combination therapies), or intraurethral suppositories. In refractory cases, penile prosthesis surgery can provide durable efficacy. Emerging or adjunctive measures in selected populations include treatment of hypogonadism with testosterone replacement when indicated, management of sleep apnea, and targeted therapy for specific vascular or neurologic conditions.
Prognosis depends on underlying cause and adherence to risk-factor management. ED should prompt a broader health assessment, because improving vascular health can enhance erectile outcomes and reduce long-term cardiovascular risk. Importantly, ED is common and treatable; stigma and avoidance can delay care, reinforcing anxiety and worsening functional performance.
