
Anxiety disorders are a group of mental health conditions characterized by excessive fear, worry, and threat-related hyperarousal that are disproportionate to the actual situation and persist over time. Clinically, anxiety becomes a disorder when it causes significant distress or impairment in social, occupational, or other important areas of functioning. Core presentations include generalized anxiety disorder (GAD) with persistent worry; panic disorder with recurrent panic attacks; social anxiety disorder with fear of scrutiny; specific phobias linked to discrete stimuli; and separation anxiety in appropriate developmental contexts. Although emotions like fear and worry can be adaptive, pathologic anxiety reflects dysregulation of threat detection, salience processing, and stress-response systems.
Neurobiologically, anxiety involves coordinated changes in the amygdala–prefrontal circuitry and related limbic networks. The amygdala is central to rapid threat appraisal, while the prefrontal cortex and anterior cingulate modulate emotional responses and support cognitive control. In anxiety disorders, functional connectivity and inhibitory control can be altered, leading to heightened threat sensitivity and difficulty down-regulating fear responses. Neurotransmitter systems also contribute. Serotonergic modulation influences mood and anxiety regulation, while noradrenergic pathways support arousal and vigilance. GABAergic interneuron dysfunction may reduce inhibitory “brakes,” contributing to sustained hyperexcitability. Dysregulation of stress hormones and the hypothalamic–pituitary–adrenal (HPA) axis has been observed in many patients, with some showing atypical cortisol dynamics and altered stress reactivity.
At the cognitive level, anxiety disorders are sustained by maladaptive threat interpretations and cognitive biases. In GAD, worry is often conceptualized as a cognitive avoidance strategy: individuals generate repetitive, abstract concerns to reduce perceived risk, but the worry paradoxically maintains arousal and impairs flexible problem-solving. In social anxiety disorder, fear centers on negative evaluation and self-focused attention, with post-event rumination reinforcing beliefs of failure or rejection. Panic disorder reflects catastrophic misinterpretation of bodily sensations (e.g., palpitations) leading to spiraling fear and subsequent panic symptoms.
Diagnostic assessment relies on clinical interviews using DSM-5-TR criteria and careful exclusion of medical and substance-related causes. Symptoms must not be better explained by other disorders, and they should occur more days than not for a sustained period in GAD. Differential diagnosis is essential: hyperthyroidism, cardiac arrhythmias, medication side effects (e.g., stimulants), and substance intoxication or withdrawal can mimic anxiety presentations. Comorbidity is common, with depression, substance use, and insomnia frequently co-occurring, which can worsen functional outcomes and treatment response if not addressed.
Evidence-based treatment is multimodal and typically includes psychotherapy, pharmacotherapy, lifestyle interventions, and management of comorbid sleep disorders. First-line psychotherapy for most anxiety disorders is cognitive behavioral therapy (CBT), which includes psychoeducation, cognitive restructuring, and exposure-based techniques. Exposure helps extinguish fear associations and recalibrate threat expectations by repeatedly confronting feared stimuli in a controlled and safety-supported manner. For GAD, CBT often targets worry processes, intolerance of uncertainty, and behavioral avoidance through applied problem-solving and delayed worry strategies.
Pharmacotherapy commonly uses selective serotonin reuptake inhibitors (SSRIs) and serotonin–norepinephrine reuptake inhibitors (SNRIs) as first-line medications. These agents reduce anxiety by modulating serotonergic and noradrenergic signaling over time, with therapeutic effects typically emerging after several weeks. In selected cases, short-term benzodiazepines may be considered for acute symptom relief; however, they carry risks of sedation, cognitive impairment, dependence, and withdrawal, so they require careful risk–benefit evaluation and time-limited use.
For refractory cases or specific phenotypes, additional options may include buspirone for GAD, certain anticonvulsants in targeted situations, or augmentation strategies guided by specialist evaluation. Sleep restoration is clinically important because insomnia amplifies hyperarousal and can maintain worry cycles. Behavioral recommendations include regular sleep timing, reduction of caffeine or other stimulants, and structured physical activity, all of which influence stress responsiveness and baseline arousal.
Prognosis is variable but often improved with timely, evidence-based intervention. Early recognition reduces chronicity, and engagement in CBT with appropriate pharmacotherapy can substantially improve symptom severity, functional recovery, and quality of life. Ongoing monitoring is necessary to assess residual symptoms, relapse risk, medication tolerability, and coexisting depression or substance use. Patient education emphasizing that anxiety disorders are treatable neurobehavioral conditions—not character flaws—supports adherence and empowers recovery.
