Cognitive models explain persistence through biased attention, catastrophic misinterpretation, and negative reinforcement. For example, avoidance can reduce short-term anxiety but prevents extinction learning, maintaining fear over time. In GAD, worry may be used as a cognitive control strategy to prevent perceived negative outcomes; however, it becomes excessive and uncontrollable, producing functional impairment. In social anxiety, safety behaviors (e.g., avoiding eye contact, rehearsing conversations) can dampen immediate fear while paradoxically preserving the belief that feared outcomes are likely.
Differential diagnosis is essential because anxiety symptoms overlap with medical and substance-related conditions. Evidence-based treatment is multimodal and tailored to symptom type, severity, comorbidity, and patient preferences. First-line psychotherapy includes cognitive behavioral therapy (CBT), which addresses maladaptive thought patterns and behaviors, and exposure-based interventions that facilitate extinction learning. For panic disorder, interoceptive exposure can reduce fear of bodily sensations. For social anxiety, CBT incorporates cognitive restructuring and graded exposure to social situations. Pharmacotherapy commonly uses selective serotonin reuptake inhibitors (SSRIs) and serotonin–norepinephrine reuptake inhibitors (SNRIs), which modulate threat learning and affect regulation. Benzodiazepines may provide rapid short-term relief by enhancing GABAergic inhibition, but they carry risks including sedation, falls, cognitive impairment, dependence, and withdrawal; thus, they are generally used cautiously, time-limited, and as an adjunct in selected cases.
Other options include buspirone for GAD, and for select patients with refractory symptoms, additional strategies such as pregabalin (where appropriate), augmentation, or referral to specialty care. Psychosocial interventions should be complemented by lifestyle measures that support autonomic stability: regular sleep timing, reduction of caffeine and stimulants, structured physical activity, and mindfulness-based approaches that improve attentional control and reduce reactivity.
Because anxiety disorders commonly co-occur with depression, substance use, and chronic medical conditions, ongoing monitoring is recommended. Long-term management focuses on sustaining functional gains, preventing relapse through continued skills practice (e.g., CBT homework and exposure maintenance), and addressing barriers to adherence such as avoidance, catastrophic beliefs, and stigma.
