Panic disorder features recurrent panic attacks—abrupt surges of intense fear with somatic symptoms—and concerns about additional attacks. Phobias involve marked fear or anxiety about specific objects or situations, with immediate fear response and avoidance. Social anxiety disorder centers on fear of negative evaluation, embarrassment, or scrutiny. Posttraumatic stress disorder (PTSD) and obsessive-compulsive and related disorders involve anxiety-like distress but are diagnosed under distinct criteria due to their characteristic symptom structures.

Assessment should be comprehensive and risk-aware. Clinicians typically evaluate symptom chronology, triggers, severity, functional impact, substance or medical contributors, and comorbidities such as depression, substance use disorders, and other anxiety disorders. Differential diagnosis is essential: hyperthyroidism, arrhythmias, pheochromocytoma, medication effects (including stimulants), and substance withdrawal can mimic or exacerbate anxiety. Standardized instruments such as the GAD-7 or panic inventories can support monitoring, but diagnosis remains clinical.

Evidence-based treatment is multimodal and often begins with psychotherapy and/or pharmacotherapy depending on severity and patient preference. Cognitive behavioral therapy (CBT) is a first-line intervention for many anxiety disorders. CBT targets maladaptive beliefs (e.g., catastrophizing), teaches coping skills, and uses exposure-based techniques to reduce avoidance and safety behaviors. In exposure therapy, repeated, controlled confrontation with feared cues allows learning that the feared outcome is unlikely or manageable, reducing fear response over time. Techniques may include interoceptive exposure for panic disorder (e.g., deliberately inducing benign bodily sensations) and graded in-vivo or imaginal exposure for phobias and PTSD-related cues.

Pharmacologic options commonly include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modulate serotonergic and noradrenergic systems implicated in threat processing. Benzodiazepines can provide short-term relief but carry risks such as tolerance, dependence, sedation, impaired cognition, and withdrawal; therefore, they are typically used cautiously and often as bridging therapy while longer-term treatments take effect. For select cases, additional strategies may be considered under specialist care.

Lifestyle and supportive interventions complement primary treatment. Regular sleep, exercise, and reduction of stimulants (excess caffeine, nicotine) can reduce physiological arousal. Stress management through mindfulness, breathing retraining, and structured problem-solving can lower baseline tension and improve emotional regulation. However, supportive measures are not substitutes for guideline-based care when symptoms are persistent or impairing.

Prognosis is generally favorable with appropriate treatment, though anxiety disorders may recur if underlying cognitive and behavioral maintaining factors are not addressed. Relapse prevention focuses on continued skills use, exposure maintenance, and early intervention when symptoms re-emerge.
