Prognosis varies with etiology, insight, comorbidities, and treatment adherence. Paranoid ideas tied to anxiety tend to improve when cognitive bias loops are interrupted and hyperarousal is reduced. Conversely, persistent, fixed delusional beliefs or comorbid psychotic disorders may require longer-term, multimodal management. Education for patients and families should emphasize that ambiguity in social life is normal, that threat predictions can be recalibrated, and that avoidance and reassurance-seeking can inadvertently reinforce suspicious learning.
