Chronic stress can sensitize the hypothalamic-pituitary-adrenal (HPA) axis, altering cortisol dynamics and reinforcing hypervigilance. Cognitively, anxiety disorders are maintained by maladaptive interpretations of bodily sensations and ambiguous cues. For example, in GAD, repetitive worry functions as a cognitive control strategy intended to reduce perceived risk, but it paradoxically sustains anxiety by preventing habituation and reinforcing threat beliefs. In panic disorder, interoceptive fear and catastrophic misinterpretation of benign physiological changes (e.g., palpitations) can generate a self-reinforcing panic cycle. In social anxiety disorder, performance fears and negative self-evaluation promote avoidance and safety behaviors that prevent disconfirming experiences.

Behaviorally, avoidance reduces short-term distress but maintains the disorder by blocking exposure to feared cues. Over time, avoidance may generalize, leading to restricted daily life and increased reliance on reassurance or safety signals. This behavioral maintenance is central to modern treatments.

Diagnosis relies on clinical evaluation using structured criteria. Clinicians assess symptom duration, intensity, triggers, functional impairment, and exclusion of medical or substance-related causes. Differential diagnosis is crucial, as thyroid disease, arrhythmias, medication side effects, caffeine or stimulant use, and sleep disorders can mimic anxiety symptoms. Comorbid depression, substance use, and trauma-related disorders are common and influence prognosis and treatment planning.

Treatment is typically multimodal. First-line psychotherapeutic approaches include cognitive-behavioral therapy (CBT), which targets maladaptive thought patterns and teaches coping skills. CBT often incorporates exposure therapy, particularly for phobias, panic disorder, and social anxiety disorder, using graded confrontation with feared sensations or situations while reducing safety behaviors. Mindfulness-based cognitive strategies can improve emotion regulation and reduce cognitive fusion with worry.

Pharmacotherapy may be indicated for moderate to severe symptoms, functional impairment, or when psychotherapy access is limited. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used as maintenance-capable agents. Benzodiazepines may provide short-term symptom relief but carry risks of dependence, sedation, cognitive effects, and should be used cautiously and generally for limited durations. For specific syndromes like panic disorder, careful titration is used to minimize early activation.

Integrated care also addresses lifestyle and comorbidities: sleep hygiene, regular physical activity, reduction of caffeine and other stimulants, and management of pain or gastrointestinal conditions. Evidence supports that improving sleep and reducing stress reactivity can attenuate symptom burden.

Prognosis varies by diagnosis, severity, comorbidity, and treatment adherence. Early intervention and sustained CBT skills often yield durable improvements. However, without treatment, anxiety disorders may become chronic, with escalating avoidance and increased risk for depression and impaired occupational and social functioning.
