
Insomnia is a common clinical condition characterized by difficulty initiating sleep (sleep-onset insomnia), maintaining sleep (sleep-maintenance insomnia), or experiencing early morning awakening, with resultant impairment in daytime functioning. Although insomnia is often portrayed as a mere inconvenience, it frequently reflects underlying neurobiological dysregulation, particularly within threat-processing and arousal systems. A central mechanistic link is persistent anxiety, in which cognitive worry and somatic tension activate hyperarousal pathways that make the transition into sleep progressively harder.
From a neurophysiological standpoint, anxiety states bias the brain toward vigilance. The amygdala and related threat-detection circuits can enhance salience of negative or uncertain information, while prefrontal systems attempt to regulate these signals through rumination and worry. When worry becomes repetitive and uncontrollable, it can prolong cognitive arousal at bedtime and reduce the likelihood that sleep-related networks dominate. This hyperarousal is measurable as increased physiological activation: elevated sympathetic nervous system tone, heightened cortisol secretion in vulnerable individuals, and faster heart rate and muscle tension.
Insomnia can also be reinforced by a cognitive-behavioral loop. Patients often develop conditioned arousal: the bed and bedtime cues become associated with wakefulness because prior attempts at sleeping were repeatedly unsuccessful. This conditioning increases attention to internal sensations (e.g., difficulty breathing, racing thoughts, time on the clock) and promotes “safety behaviors” such as checking the time, scrolling devices, or trying to force sleep. These behaviors further disrupt circadian timing and reduce homeostatic sleep drive. Over time, the individual may experience learned helplessness about sleep, worsening anxiety and sustaining insomnia.
Clinically, anxiety-related insomnia may appear in several diagnostic configurations, including generalized anxiety disorder (GAD) and panic-related presentations, or as insomnia secondary to comorbid mood disorders. However, the symptom pathway commonly converges on two features: (1) cognitive intrusion—worry, rumination, and catastrophic thinking that competes with sleep-facilitating cognitive shutdown; and (2) physiological activation, including hyperventilation tendencies and increased cortical alertness. Even when the person is physically tired, the brain can remain in a state that resembles threat response rather than rest and recovery.
Sleep architecture can provide additional insight. Anxiety-related insomnia may reduce slow-wave sleep and fragment rapid eye movement (REM) sleep, impairing emotional regulation and memory consolidation. Disturbed sleep then increases vulnerability to anxiety through bidirectional pathways: poor sleep worsens emotion regulation, amplifies perceived threat, and can reduce tolerance for uncertainty. This bidirectionality helps explain why the cycle persists without targeted intervention.
Evidence-based management typically integrates cognitive-behavioral therapy for insomnia (CBT-I) with anxiety-focused strategies when indicated. CBT-I addresses conditioned arousal and maladaptive beliefs using components such as stimulus control (associating the bed with sleep rather than wakefulness), sleep restriction therapy (compressing time in bed to build stronger sleep drive), cognitive restructuring (challenging catastrophic interpretations of sleeplessness), and relaxation training (progressive muscle relaxation, paced breathing, or mindfulness-based approaches). For comorbid anxiety, CBT frameworks often incorporate worry exposure, problem-solving skills, and cognitive restructuring to reduce rumination.
Pharmacologic treatments may be considered in selected cases, but they are generally not first-line for long-term insomnia due to risks including tolerance, dependence (particularly with benzodiazepines), cognitive side effects, and rebound insomnia on discontinuation. When medications are used, clinicians weigh short-term symptom relief against the need for sustained behavioral change. Treating the underlying anxiety disorder—when present—can be essential; otherwise, insomnia may return despite initial improvement.
Red flags warranting medical evaluation include insomnia persisting beyond several weeks with significant functional impairment, comorbid depression or suicidal ideation, symptoms of sleep-disordered breathing (snoring with witnessed apneas), restless legs symptoms, or frequent panic attacks. Comprehensive assessment should include sleep history, medication and substance review (caffeine, nicotine, alcohol), and screening for circadian rhythm disorders.
A practical clinical principle is that insomnia is not merely the absence of sleep; it is the presence of hyperarousal—cognitive, emotional, and physiological. Addressing anxiety-related arousal mechanisms can restore the conditions necessary for sleep onset and maintenance. In this context, the aphorism highlights a core clinical truth: persistent worry can “keep people awake,” perpetuating a biological state in which sleep is actively inhibited.
