Clinicians should avoid pathologizing culturally sanctioned mourning practices while still identifying impairing symptoms.

Evidence-based treatment for complicated grief focuses on restoring engagement with life while integrating the loss. Psychotherapies with the strongest evidence include Complicated Grief Therapy (CGT), which combines elements of cognitive-behavioral strategies, meaning reconstruction, and exposure-like techniques to reduce avoidance of loss-related cues. Interventions target painful cognitions (e.g., pervasive self-blame), strengthen coping skills, and facilitate adaptive remembrance. For those with co-occurring depression or anxiety, treatment may be tailored using cognitive-behavioral therapy (CBT) approaches and targeted pharmacotherapy when indicated.

Pharmacologic management is individualized and typically addresses comorbid conditions rather than treating grief per se. Antidepressants may be considered for moderate to severe MDD, persistent insomnia, or severe anxiety symptoms, particularly when suicidality, profound functional decline, or persistent dysphoria extend beyond grief expectations. However, medication alone may not resolve core PGD mechanisms such as prolonged yearning and maladaptive meaning-making. Therefore, a stepped approach—psychotherapy as the foundation, medication when comorbid syndromes are present—is often recommended.

When grief becomes clinically significant, safety assessment is essential. Risk may include suicidal ideation, especially in individuals with previous depression, substance use disorders, or traumatic loss. Clinicians should evaluate for self-harm intent, access to lethal means, and protective factors. Supportive counseling, family interventions, and coordination with primary care can mitigate functional deterioration.

Self-care and coping strategies can be adjunctive but should not replace professional care when impairment is substantial. Practical supports include regular sleep-wake scheduling, gentle activity to reduce deconditioning, structured social contact, and limiting avoidance of meaningful cues when it increases suffering. Grief can involve identity and role loss, so rebuilding routines (work, caregiving, social responsibilities) is central to recovery. Meaning-centered practices—such as rituals, storytelling, and spiritual or cultural observances—may help integrate the loss into a continuing life narrative.
