Dysregulation within cortico-striato-thalamo-cortical circuits affects threat habit learning and persistent symptom maintenance. Neurochemical contributors include enhanced noradrenergic signaling and alterations in serotonergic and GABAergic transmission, which together influence arousal, worry, and inhibitory control. Chronic stress can further sensitize stress-response pathways, including hypothalamic-pituitary-adrenal (HPA) axis activity.
Diagnostic evaluation relies on symptom patterns, duration, and associated impairment. Generalized anxiety disorder (GAD) involves excessive worry occurring more days than not for at least several months, with difficulty controlling the worry and associated symptoms such as restlessness, fatigue, concentration problems, irritability, muscle tension, and sleep disturbance. Panic disorder features recurrent unexpected panic attacks—abrupt surges of intense fear or discomfort—often followed by worry about additional attacks or behavioral avoidance. Social anxiety disorder involves intense fear of negative evaluation in social or performance situations, with avoidance or enduring distress. Specific phobias center on circumscribed fears leading to avoidance.
Cognitive mechanisms are central to many anxiety disorders. Cognitive models emphasize threat appraisal biases: individuals may interpret bodily sensations as dangerous (interoceptive misinterpretation), assume worst-case outcomes, or treat uncertainty as intolerable. Behavioral models highlight avoidance and safety behaviors, which reduce anxiety short-term but prevent corrective learning, thereby maintaining fear in the long term. For example, avoidance of triggering situations prevents exposure-based habituation and cognitive restructuring.
Physiological symptoms—palpitations, sweating, trembling, dyspnea, gastrointestinal discomfort, dizziness, and insomnia—reflect autonomic arousal mediated by sympathetic pathways. Sleep disturbance can amplify amygdala reactivity and impair prefrontal regulation the next day, creating a reinforcing cycle. Persistent worry can also increase muscle tension and heighten pain sensitivity.
Treatment is evidence-based and often multimodal. Psychotherapy is first-line for many patients. Cognitive behavioral therapy (CBT) targets distorted threat beliefs and promotes coping strategies. Exposure-based approaches help extinguish conditioned fear and rebuild contextual safety learning, especially for specific phobias and panic disorder. For GAD, CBT commonly includes worry scheduling, cognitive restructuring, problem-solving training, and interoceptive awareness techniques to reduce fear of sensations.
Pharmacotherapy includes selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modulate serotonergic and noradrenergic systems over time. These medications reduce symptom frequency and severity and support durable recovery when combined with skills-based therapy. Benzodiazepines can rapidly attenuate acute anxiety by enhancing GABA-A receptor activity, but they carry risks including sedation, tolerance, dependence, and withdrawal; thus they are generally reserved for short-term or specific clinical circumstances. Other options may be considered by clinicians based on comorbidities and patient history.
Comorbid conditions—depression, substance use disorders, and trauma-related symptoms—are common and significantly influence prognosis. Assessment should also rule out medical mimics such as hyperthyroidism, arrhythmias, medication side effects, and stimulant or substance-related anxiety. Lifestyle interventions can be adjunctive: consistent sleep, regular aerobic activity, stress management practices, and limiting caffeine and other anxiogenic substances may reduce baseline arousal. However, these do not replace targeted psychotherapy or medication when symptoms are clinically significant.
Risk factors include family history, temperament (behavioral inhibition), chronic stress exposure, childhood adversity, and reinforcement of anxious avoidance. Anxiety disorders can fluctuate, but many respond well to early, sustained treatment. Key goals are to restore adaptive threat learning, reduce avoidance, improve emotion regulation, and address functional impairment.
