
Anxiety disorders are a group of related mental health conditions characterized by excessive fear, worry, and associated behavioral and physiological symptoms that impair functioning. Although anxiety is a normal adaptive response to perceived threat, anxiety disorders involve dysregulation of threat detection and fear response, leading to persistent or disproportionate symptoms. Clinically, the unifying feature is heightened anticipatory concern coupled with patterns of avoidance or maladaptive coping.
From a neurobiological perspective, anxiety disorders reflect altered circuitry among the amygdala, hippocampus, prefrontal cortex, and related stress-response networks. The amygdala contributes to rapid threat appraisal, while the hippocampus shapes contextual memory of danger. The prefrontal cortex modulates emotional responses and safety learning; when top-down regulation is inefficient, threat interpretations can remain activated. Neurochemical systems implicated include gamma-aminobutyric acid (GABA) for inhibitory control, serotonin for mood and threat appraisal, norepinephrine for arousal and vigilance, and corticotropin-releasing factor pathways that interface with the hypothalamic-pituitary-adrenal (HPA) axis. Dysregulated HPA activity can amplify hyperarousal and somatic symptoms, perpetuating a cycle of worry and physiological activation.
Cognitively, anxiety disorders involve biased attention toward threat, overestimation of harm, intolerance of uncertainty, and catastrophic interpretations of bodily sensations. For many patients, symptoms become self-reinforcing: increased monitoring of internal states (e.g., heart rate, breathing) is interpreted as danger, which increases arousal and leads to more worry. Behavioral avoidance reduces short-term distress but prevents corrective learning that would disconfirm catastrophic expectations. Over time, this maintenance process can generalize anxiety across contexts.
Diagnostic frameworks distinguish several core anxiety disorders: generalized anxiety disorder (GAD), panic disorder, specific phobias, social anxiety disorder, and agoraphobia; related entities include separation anxiety disorder and selective mutism (typically in childhood). GAD is characterized by excessive anxiety and worry occurring more days than not for at least several months, difficult to control, and accompanied by symptoms such as restlessness, fatigue, concentration difficulties, irritability, muscle tension, or sleep disturbance. Panic disorder involves recurrent unexpected panic attacks with persistent concern or behavioral change related to additional attacks. Phobic disorders involve marked fear of specific objects or situations with immediate fear response, avoidance or endurance with intense distress, and disproportionate fear. Social anxiety disorder features fear of scrutiny or negative evaluation in social or performance situations. Agoraphobia centers on fear and avoidance of situations where escape might be difficult or help unavailable.
Epidemiologically, anxiety disorders are common and frequently co-occur with depressive disorders, substance use, and insomnia. Comorbidity is clinically important because shared mechanisms—such as negative affectivity, stress-reactivity, and maladaptive cognitive patterns—may maintain multiple symptom domains. Risk factors include a family history of anxiety or depression, early adversity, chronic medical illness, and temperamentally inhibited behavior. Medical causes and medication effects should also be considered when evaluating anxiety-like symptoms; hyperthyroidism, stimulant use, and certain respiratory conditions can mimic or exacerbate anxiety.
Treatment is multimodal and evidence-based. Psychotherapy is a first-line option across many anxiety disorders, particularly cognitive behavioral therapy (CBT). CBT targets cognitive distortions, teaches skills for emotion regulation, and incorporates graded exposure to feared cues to facilitate inhibitory learning and reduce avoidance. For panic disorder, CBT often includes interoceptive exposure to feared sensations (e.g., dizziness, palpitations) combined with cognitive restructuring. For GAD, CBT focuses on worry control strategies, problem-solving, and reducing time spent in unproductive rumination. Acceptance-based interventions and mindfulness approaches can complement CBT by reducing experiential avoidance.
Pharmacotherapy is effective for several anxiety disorders, especially when symptoms are severe, chronic, or functionally impairing. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used due to favorable long-term efficacy and tolerability profiles. Benzodiazepines can reduce acute symptoms but carry risks of sedation, cognitive impairment, dependence, and withdrawal; therefore, they are typically reserved for short-term or carefully selected situations. Buspirone may be used for GAD in some cases. Medication choice should consider symptom profile, comorbid depression, prior treatment response, pregnancy considerations, drug interactions, and patient preferences.
A comprehensive care plan also includes lifestyle and medical management. Sleep optimization, regular physical activity, caffeine and substance moderation, and stress management can reduce physiological vulnerability. Clinicians should monitor for suicidal ideation in comorbid depression, address trauma-related symptoms when present, and integrate social supports. Prognosis is generally favorable with appropriate treatment, though relapse prevention requires maintenance strategies such as continued practice of exposure principles, cognitive skills, and adherence to follow-up.
