Eating Disorders: Pathophysiology, Long-Term Complications, Recovery, and Sudden Cardiopulmonary Risk

Eating disorders are serious psychiatric illnesses characterized by persistent disturbances in eating behavior and clinically significant distress or impairment. The major syndromes include anorexia nervosa, bulimia nervosa, and binge-eating disorder, each with distinctive behavioral patterns but overlapping medical risks. The core clinical feature across these conditions is dysregulated nutritional intake—ranging from restrictive energy consumption to recurrent episodes of binge eating—often accompanied by compensatory behaviors in anorexia nervosa (e.g., excessive exercise) and bulimia nervosa (e.g., purging). Despite differences in symptom presentation, the unifying medical theme is that disordered eating can produce profound, multi-system physiologic compromise.

Anorexia nervosa typically involves energy restriction leading to weight loss and malnutrition. The body adapts by downregulating metabolic processes, altering endocrine signaling, and impairing immune function. Chronic undernutrition produces electrolyte abnormalities, including hypokalemia, hypomagnesemia, and hypophosphatemia, which increase neuromuscular irritability and create an arrhythmogenic substrate. Malnutrition also affects the cardiovascular system: reduced cardiac muscle mass, diminished cardiac output, and prolonged repolarization on electrocardiography can occur. In addition, autonomic dysfunction and altered baroreflex sensitivity may increase susceptibility to syncope and fatal arrhythmias. Long-term complications of eating disorders extend beyond the heart. Gastrointestinal complications include delayed gastric emptying, reflux, constipation, and in severe cases, bowel motility impairment. Endocrine disruptions can cause amenorrhea, infertility, and reduced bone mineral density through suppression of gonadotropins and alterations in the hypothalamic-pituitary-gonadal axis. Bone loss progresses to osteopenia or osteoporosis, increasing fracture risk. Hematologic effects include anemia and leukopenia. Neurologic and psychiatric effects include cognitive slowing, fatigue, and increased risk of depression, anxiety disorders, and substance misuse. Mortality risk is elevated in both restrictive and purging subtypes due to medical complications and, in some cases, suicide.

A critical, clinically actionable concept is the medical fragility during recovery. Even when intake begins to improve, the body can experience metabolic instability. Refeeding after prolonged undernutrition can precipitate refeeding syndrome, a potentially fatal shift in fluids and electrolytes driven by restoring carbohydrate metabolism. Insulin secretion increases, driving phosphate, potassium, and magnesium into cells, which can cause respiratory failure, hemolysis, cardiac arrhythmias, seizures, and neurologic dysfunction. Therefore, evidence-based refeeding requires careful monitoring, gradual caloric escalation, and proactive supplementation of electrolytes, typically under inpatient or closely supervised outpatient protocols depending on risk.

Cardiac complications deserve particular attention because they can occur during both chronic illness and vulnerable transition periods. Electrolyte depletion (especially potassium and magnesium), volume depletion, oxidative stress, and structural cardiomyopathy create conditions for sudden cardiac events. Some individuals may have no prominent warning symptoms before a catastrophic arrhythmia. This risk underscores the need for integrated care: psychiatric treatment should be paired with medical surveillance (vital signs, weight trends, metabolic panels, and electrocardiography when indicated) and assessment for complications.

Treatment frameworks emphasize recovery-oriented, multidisciplinary management. Psychotherapies with evidence support include cognitive behavioral therapy for bulimia nervosa and binge-eating disorder, and family-based therapy for adolescents with anorexia nervosa. Nutritional rehabilitation is central, aiming for safe weight restoration and normalization of eating patterns while addressing fear of weight gain and cognitive distortions. Pharmacotherapy may be used adjunctively: for example, antidepressants such as selective serotonin reuptake inhibitors can help in bulimia nervosa and comorbid depressive or anxiety symptoms, though they do not replace nutritional and psychotherapeutic care. In anorexia nervosa, medications are not a substitute for weight restoration, but they can target comorbidities and symptom burden.

Importantly, “being in recovery” does not eliminate medical risk immediately. Arrhythmia susceptibility can persist due to lingering cardiac remodeling, ongoing electrolyte fragility, and the physiologic aftereffects of prolonged malnutrition. Clinically, this means that patients and clinicians must treat recovery as a dynamic medical process requiring monitoring—especially early in treatment, after relapses, or during rapid nutritional changes. When sudden deterioration occurs, it should prompt emergent evaluation for arrhythmia, electrolyte disturbance, dehydration, infection, and refeeding-related metabolic derangements.

Ultimately, eating disorders are not solely behavioral problems; they are neuropsychiatric illnesses with direct, potentially fatal biological consequences. Public discussions should focus on accurate medical understanding: that persistent malnutrition and/or purging can lead to lethal complications; that recovery requires structured medical support; and that sudden cardiac and cardiopulmonary events can occur even during transition periods.
