Behavioral interventions can specifically target the cognitive and emotional drivers of avoidance. Techniques include goal setting using “process goals” (e.g., time spent moving) rather than solely weight outcomes, self-monitoring, stimulus control, and cognitive restructuring of catastrophic beliefs (“I’ll be judged”) into realistic appraisals (“I can start small and adapt”). Exposure-based methods can reduce fear of embarrassment by incrementally engaging in feared activities at manageable intensity.

When progress plateaus or obesity-related comorbidities are present, medications may be appropriate. Options that modulate appetite pathways or gut-brain signaling can support weight loss when paired with lifestyle changes. Care must include assessment for contraindications and monitoring of adverse effects. Finally, it is crucial to frame fitness promotion as supportive health care rather than moral judgment.
