Substance-related disorders and mood disorders with anxious distress must also be distinguished, as treatment selection depends on accurate diagnosis.

Treatment is evidence-based and often multimodal. First-line psychotherapy includes cognitive behavioral therapy (CBT), which targets maladaptive thought patterns and behavioral avoidance through cognitive restructuring and graded exposure. Exposure therapy is particularly effective when fear is circumscribed (e.g., phobias, social anxiety) and can be adapted for GAD by emphasizing tolerance of uncertainty and worry interruption techniques. Acceptance-based approaches (such as ACT) can help patients reduce experiential avoidance by shifting from suppressing anxiety to changing the relationship with internal sensations.

Pharmacotherapy is commonly indicated for moderate-to-severe symptoms, comorbidities, or when rapid symptom reduction is needed. SSRIs and SNRIs are standard for many anxiety disorders due to their effect on serotonergic and noradrenergic signaling. Benzodiazepines can reduce acute anxiety by enhancing GABAergic inhibition, but they carry risks of sedation, dependence, and impaired cognition; guidelines generally recommend limiting duration and using them selectively. For panic disorder, careful titration and maintenance strategies are used to avoid early activation. Additional options may include buspirone for GAD in some cases, and beta-blockers for performance-related anxiety, though they do not address cognitive mechanisms.

Lifestyle and adjunctive interventions can support recovery but are rarely sufficient alone. Regular aerobic activity improves stress resilience, while sleep stabilization reduces hyperarousal and lowers threat sensitivity. Mindfulness practices may reduce attentional bias to threat cues. Psychoeducation is critical: it helps patients understand anxiety as a false alarm system rather than an immediate danger signal, improving engagement in CBT and exposure tasks.

Long-term outcomes improve when patients receive accurate diagnosis, consistent therapy, and relapse-prevention planning. Relapse can occur when avoidance returns or when medication is discontinued abruptly. A sustained strategy typically includes continued CBT skills practice, gradual re-engagement with avoided activities, and monitoring for comorbid depression, which can worsen prognosis.
