Ceasefire-Related Stress, Hyperarousal, and Aggressive Urges: Mental Health Impacts of Perceived Threat Exposure

Ceasefire announcements in conflict settings can acutely alter threat appraisal and trigger stress responses in civilians and service members alike. Even when a ceasefire is intended to reduce violence, individuals may experience intensified vigilance, intrusive thoughts, and heightened emotional arousal driven by uncertainty, rumor, and perceived risk. From a clinical perspective, this cluster reflects activation of the brain’s threat circuitry—particularly the amygdala-centered salience network—followed by compensatory attempts at emotion regulation that may fail under sustained uncertainty.

A ceasefire can reduce overt threat yet increase ambiguity, which is a powerful driver of anxiety-related physiology. When the brain cannot reliably predict whether violence will resume, it tends to maintain a high baseline state of hypervigilance. This resembles anxiety mechanisms seen in generalized anxiety and post-traumatic stress symptomatology: attention is biased toward danger cues, working memory is consumed by threat monitoring, and cognitive load increases. Neurobiologically, stress involves dysregulation of the hypothalamic–pituitary–adrenal (HPA) axis, with altered cortisol dynamics and autonomic arousal mediated by the sympathetic nervous system. Behaviorally, hyperarousal can be expressed as irritability, sleep disruption, and aggressive or punitive ideation.

In some individuals, perceived threat and moral outrage can amplify what clinicians term “threat-based emotion processing.” Anger often serves as an approach-oriented emotion that motivates action. However, when anger is tightly coupled to fear and uncertainty, it may promote impulsive decision-making. The cognitive model of threat emphasizes appraisal: if a person interprets a ceasefire as strategically unsafe or as allowing adversaries time to regroup, they may experience heightened anticipatory anxiety and a sense of urgency. Urgent cognition can shorten the psychological time horizon, increasing preference for immediate, forceful interventions rather than slower conflict de-escalation.

This is clinically relevant because prolonged exposure to threat cues—whether direct or via media—can produce symptoms overlapping with post-traumatic stress disorder (PTSD). Common PTSD mechanisms include intrusive memories, hyperarousal (e.g., exaggerated startle, irritability), and negative alterations in cognitions and mood (e.g., persistent expectation of harm). Even without a formal PTSD diagnosis, repeated cycles of perceived danger and relief followed by uncertainty can maintain an anxious arousal loop. The result may be a form of adjustment-related disturbance characterized by disturbed sleep, concentration problems, and persistent worry.

Aggressive urges can also be understood through the lens of impaired top-down control. Stress depletes executive resources in the prefrontal cortex, reducing capacity for inhibitory control and cognitive reappraisal. Under high arousal, the amygdala’s influence on attention and behavior becomes more dominant, favoring rapid, emotion-congruent responses. When individuals are instructed or encouraged to endorse violent language, the risk of normalizing dehumanizing narratives increases; dehumanization can lower empathy and moral constraints, which may intensify aggressive ideation.

Importantly, clinicians consider how online discourse contributes to symptom persistence. Social media can act as a continuous “threat amplifier” by delivering rapidly repeating claims, counterclaims, and emotionally salient content. This can sustain stress physiology and reinforce biased interpretation. Exposure also interferes with recovery by disrupting sleep and preventing psychological disengagement.

Effective interventions target both physiology and cognition. For anxiety and PTSD-spectrum symptoms, evidence-based strategies include trauma-focused psychotherapy and anxiety-focused cognitive behavioral therapy (CBT). CBT helps reframe threat appraisals, reduce safety behaviors driven by fear, and challenge catastrophic interpretations. For hyperarousal and sleep, techniques such as stimulus control, sleep hygiene, and—when appropriate—brief relaxation training can restore autonomic balance. Pharmacologic options may include selective serotonin reuptake inhibitors (SSRIs) for anxiety/PTSD symptoms; for acute insomnia or severe distress, clinicians may consider short-term, carefully monitored approaches, recognizing risks of sedation and dependence.

At the community level, accurate information and uncertainty-reducing communication are key. Clear ceasefire monitoring details, credible verification mechanisms, and consistent messaging can reduce ambiguity-driven anxiety. In parallel, public health approaches emphasize psychological first aid: validation of distress, practical support, and guidance to reduce exposure to destabilizing information cycles.

When stress escalates into urges for punitive or violent action, immediate risk assessment is warranted. Mental health professionals assess for acute danger to self or others, substance use, history of violence, command hallucinations, and severe agitation. If a person expresses intent or perceives imminent threat coupled with loss of control, urgent intervention and crisis resources are appropriate.

In summary, ceasefire-related uncertainty can precipitate hyperarousal, intrusive cognition, and anger-fear coupling through well-characterized stress and threat mechanisms. Recognition of these processes supports targeted assessment and evidence-based interventions to reduce suffering, improve emotion regulation, and enhance safe, constructive coping during periods of unstable conflict.
