Panic disorder involves recurrent unexpected panic attacks followed by persistent concern about additional attacks and/or maladaptive behavior to avoid triggers. Social anxiety disorder features marked fear of social or performance situations due to potential embarrassment, with avoidance or endurance with significant distress. Specific phobias involve circumscribed fear related to particular stimuli, often with immediate anxiety response and avoidance.
A careful assessment is essential because anxiety symptoms may stem from medical conditions (e.g., hyperthyroidism, arrhythmias, substance/medication effects) or other psychiatric disorders (e.g., depressive disorders, PTSD, OCD). Substance use can mimic or worsen anxiety via stimulants or withdrawal syndromes. Clinicians also evaluate comorbidities, since anxiety frequently co-occurs with depression, insomnia, and certain neurodevelopmental conditions, affecting treatment choice and prognosis.
Evidence-based treatment integrates psychotherapy, pharmacotherapy, and self-management. First-line psychotherapy for many anxiety disorders includes cognitive-behavioral therapy (CBT). CBT targets maladaptive threat interpretations, catastrophic thinking, and avoidance behaviors through cognitive restructuring and behavioral experiments. Pharmacologic options depend on diagnosis and symptom profile. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used due to efficacy and favorable long-term risk profiles. Benzodiazepines can provide short-term relief through enhanced GABA-A mediated inhibition, but they carry risks including sedation, falls, tolerance, and dependence; therefore, they are typically limited to brief periods or specific circumstances. For acute severe anxiety, careful monitoring and gradual initiation strategies may be used, as early treatment can transiently increase anxiety in some patients. Adjuncts such as sleep-focused interventions, stress management, and, in select cases, other agents under specialist care may be considered.
Lifestyle factors support symptom reduction but are not substitutes for first-line care. Regular physical activity reduces stress reactivity and improves sleep continuity. Limiting caffeine and other stimulants can attenuate somatic hyperarousal. Mindfulness and relaxation practices may reduce physiological tension and facilitate cognitive flexibility. However, persistent or disabling anxiety warrants structured evaluation and treatment planning.
Prognostically, anxiety disorders are treatable, especially when addressed early and with evidence-based interventions. Combining CBT with pharmacotherapy can be beneficial for moderate-to-severe cases or when a patient has difficulty engaging in therapy alone. Relapse prevention emphasizes ongoing skills practice, gradual reduction of avoidant behaviors, and maintenance psychotherapy or medication strategies when indicated.
