A common cognitive mechanism across anxiety disorders is attentional bias toward threat and catastrophizing—interpreting ambiguous bodily sensations or external cues as signs of impending harm. In GAD, worry is often pervasive, difficult to control, and accompanied by symptoms such as restlessness, fatigue, impaired concentration, irritability, muscle tension, and sleep disturbance. Cognitive models propose that intolerance of uncertainty drives repetitive verbal thinking about potential negative outcomes. This worry functions as an attempted coping strategy (e.g., mental simulation to prevent bad events) but paradoxically maintains anxiety through avoidance of emotional processing and reinforcement of threat beliefs.
Physiologically, anxiety disorders trigger activation of the sympathetic nervous system and stress axes. Patients may experience palpitations, sweating, tremor, gastrointestinal discomfort, shortness of breath, or dizziness. These symptoms reflect heightened arousal and can create a self-perpetuating cycle: bodily sensations increase threat interpretation, which further increases anxiety and arousal. In panic disorder, recurrent unexpected panic attacks occur, and subsequent maladaptive behaviors (avoidance of activities, safety behaviors, or hypervigilance to bodily sensations) can sustain the disorder.
Diagnostic evaluation requires careful clinical assessment to distinguish anxiety disorders from medical conditions (e.g., hyperthyroidism, arrhythmias, substance/medication effects) and from other psychiatric diagnoses (e.g., depressive disorders, trauma-related disorders, bipolar disorders). Clinicians consider symptom duration, severity, triggers, functional impairment, and rule out differential diagnoses. For example, phobias are linked to specific feared objects or situations and lead to targeted avoidance, whereas GAD involves more generalized, chronic worry across domains.
Evidence-based treatment integrates psychotherapy, pharmacotherapy when indicated, and lifestyle or behavioral interventions. First-line psychotherapy for many anxiety disorders is cognitive behavioral therapy (CBT). CBT targets maladaptive thought patterns and behaviors through cognitive restructuring, exposure-based techniques, and skills training. Exposure therapy is particularly important for phobias and panic-related avoidance; repeated, structured confrontation with feared cues reduces fear through new learning and extinction of conditioned responses.
Acceptance-based and mindfulness-oriented approaches can be beneficial, especially when worry is driven by experiential avoidance. These therapies aim to reduce fusion with catastrophic thoughts and increase tolerance of internal sensations. Pharmacologic options often include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modulate serotonergic and noradrenergic signaling, helping reduce baseline anxiety and reactivity. Benzodiazepines may provide short-term symptom relief, particularly in acute phases, but they carry risks including sedation, cognitive impairment, dependence, and withdrawal; therefore, they are typically used cautiously and for limited durations.
When treating anxiety disorders, clinicians should also address sleep, caffeine or stimulant use, alcohol patterns, exercise, and comorbid conditions such as insomnia or depression. Gradual reduction of avoidance behaviors and implementation of coping strategies (problem solving, relaxation training, and stress management) can improve outcomes. Long-term prognosis is generally favorable with appropriate, sustained treatment, though relapses can occur and may require booster sessions or medication adjustments.
Because anxiety disorders can overlap with cardiovascular symptoms and gastrointestinal complaints, early recognition and comprehensive assessment are crucial. Patients benefit from psychoeducation explaining the biological basis of arousal, the role of cognitive threat appraisals, and the effectiveness of structured therapy and medications.
