Anxiety is a neurobiological and psychological state characterized by excessive fear, apprehension, or worry accompanied by bodily arousal. Neurobiologically, anxiety involves an imbalance between threat-detection networks and inhibitory control networks. When top-down control is weak or when threat signals are amplified, anxious thoughts become sticky and the body remains in heightened readiness. The limbic system interacts with hypothalamic and brainstem circuits, activating the autonomic nervous system and the hypothalamic–pituitary–adrenal (HPA) axis. This drives release of stress mediators such as cortisol and noradrenaline, which increase vigilance, reduce sleep quality, and can intensify physical sensations (e.g., shortness of breath, tremor), creating a feedback loop where bodily symptoms reinforce catastrophic interpretations.

Cognitively, many patients develop maladaptive interpretations of symptoms. For example, an elevated heart rate may be appraised as danger (“something is seriously wrong”), leading to further anxiety and increased physiological arousal. This cycle is prominent in panic disorder, generalized anxiety disorder (GAD), and health anxiety. In GAD, worry is typically generalized and difficult to control, occurring more days than not for at least several months, and it is associated with symptoms such as restlessness, fatigue, difficulty concentrating, irritability, muscle tension, and sleep disturbance. In contrast, specific phobias involve marked fear of circumscribed stimuli with avoidance patterns. Social anxiety disorder centers on fear of negative evaluation and embarrassment, often leading to performance avoidance and safety behaviors.

Diagnostic evaluation requires careful differentiation from medical conditions that mimic anxiety, including hyperthyroidism, arrhythmias, pheochromocytoma, substance- or medication-induced states (stimulants, withdrawal from alcohol/benzodiazepines), and sleep disorders. Clinicians also assess comorbid depression, which commonly co-occurs and can worsen prognosis. Substance use screening is essential because caffeine, nicotine, and illicit stimulants can both precipitate and maintain anxious arousal.

Evidence-based treatment typically combines psychotherapy, medications, and lifestyle interventions. Cognitive behavioral therapy (CBT) is first-line for most anxiety disorders. CBT targets cognitive distortions, teaches skills for challenging catastrophic beliefs, and uses exposure-based techniques to reduce fear through habituation and inhibitory learning. For GAD, CBT often includes worry management, cognitive restructuring, and training in problem-solving and mindfulness-based strategies to reduce rumination. Exposure therapy is particularly effective in phobias and panic disorder by systematically reducing avoidance and safety behaviors that prevent learning that feared outcomes are unlikely.

Pharmacotherapy may be used when symptoms are severe, persistent, or when psychotherapy alone is insufficient. Selective serotonin reuptake inhibitors (SSRIs) and serotonin–norepinephrine reuptake inhibitors (SNRIs) are commonly recommended as first-line agents for GAD, social anxiety disorder, and panic disorder. They typically require several weeks to show benefit and can initially increase anxiety in some individuals, which is why gradual titration and close follow-up are important. Short-term benzodiazepines may be considered selectively for acute symptom relief, but risks include sedation, tolerance, cognitive effects, and dependence; therefore they are generally not preferred as long-term therapy.

Adjunctive strategies include sleep optimization, regular aerobic exercise, reducing caffeine and alcohol, and structured stress-management practices. Because anxiety is maintained by threat appraisal and physiological arousal, interventions that reduce hyperarousal—such as diaphragmatic breathing, progressive muscle relaxation, and consistent sleep schedules—can improve symptom control and resilience. Mindfulness-based approaches can reduce experiential avoidance and improve tolerance of internal sensations.

Prognosis is generally favorable with appropriate treatment, especially when therapy addresses both cognitive and behavioral maintaining factors. Early intervention improves functional outcomes. Persistent anxiety can increase risk for depression, substance misuse, and cardiovascular concerns through chronic stress physiology, underscoring the importance of comprehensive, evidence-based care.
