This is a key reason food insecurity is associated with reduced treatment adherence in chronic disease (e.g., diabetes, hypertension), delayed healthcare seeking, and poorer outcomes in both physical and mental health conditions.

Food insecurity intersects with public health and social determinants. Structural factors such as unemployment, low wages, unstable housing, high food prices, limited social protection, and migration-related barriers can sustain deprivation. In many settings, households may cope by reducing meal frequency, decreasing portion sizes, switching to lower-cost food with inferior nutritional quality, or relying on informal support networks. While these coping strategies can temporarily reduce hunger, prolonged reliance can intensify nutritional harm.

Clinically, assessment of food insecurity is feasible using validated screening questions. Tools such as the US Department of Agriculture 6-item module or short survey instruments used internationally ask about reduced food intake, skipping meals, and financial constraints related to food. Screening is typically recommended in primary care, maternal health, and chronic disease programs, because identification enables targeted interventions.

Evidence-based interventions include both individual-level and system-level actions. At the individual level, access to nutritious food through food pantries, medically tailored meal programs, and referral to social services can improve dietary quality. For high-risk groups, integration with nutrition counseling and symptom management is crucial. At the system level, policies that improve income supports (cash transfers), strengthen unemployment and disability benefits, subsidize healthy foods, and reduce housing instability can reduce prevalence and duration. Community-based strategies—such as school feeding programs, emergency food distribution with nutrition standards, and mobile pantry services—can mitigate acute crises.

Healthcare teams can adopt practical steps: screen routinely, document results in the medical record, coordinate with dietitians and social workers, and connect patients to benefits (e.g., food assistance programs where available). For pregnant patients and children, timely interventions are essential to prevent adverse developmental outcomes. For adults with diabetes or cardiovascular disease, ensuring consistent access to appropriate carbohydrates and protein can improve glycemic control and reduce complications.

In summary, food insecurity is a medically significant exposure that drives a multi-system cascade: stress hormone dysregulation, immune vulnerability, micronutrient deficiency, metabolic instability, and elevated risk of anxiety and depression. Addressing it requires both clinical screening and upstream policy action to ensure reliable access to adequate, nutritious food.
