
Anxiety disorders are a group of mental health conditions characterized by excessive fear, worry, and related behavioral disturbances that impair functioning. Unlike transient stress responses that often resolve as a threat passes, pathological anxiety persists or escalates, is disproportionate to circumstances, and can become entrenched through cognitive, behavioral, and neurobiological reinforcement.
Core clinical features include persistent or recurrent worry (often for multiple domains) accompanied by symptoms such as restlessness, fatigue, difficulty concentrating, irritability, muscle tension, and sleep disturbance. Patients may also experience panic attacks—abrupt surges of intense fear that peak within minutes—often with palpitations, shortness of breath, chest discomfort, dizziness, and derealization or fear of losing control. Avoidance is a common maintaining factor: individuals may skip situations that trigger anxiety, which temporarily reduces distress but prevents disconfirmation of catastrophic beliefs.
From a mechanistic perspective, anxiety involves dysregulation across fear circuitry, stress-response systems, and cognitive appraisal. Functional brain and neurocircuit findings implicate heightened reactivity in the amygdala and altered regulation by prefrontal cortical regions that normally inhibit threat responses. The locus coeruleus–noradrenergic system and hypothalamic–pituitary–adrenal (HPA) axis can contribute to hyperarousal, with elevated baseline sympathetic tone and abnormal cortisol signaling in some individuals. Learning processes also matter: classical conditioning can link neutral cues with threat, while operant conditioning can reward avoidance behaviors, strengthening anxiety through negative reinforcement.
Cognitively, many anxiety disorders involve attentional bias toward threat and interpretive bias that favors danger-related explanations. Catastrophic misinterpretation (e.g., “This bodily sensation means I will collapse or die”) amplifies physiological arousal and triggers further worry. Metacognitive beliefs—such as the notion that worry is uncontrollable or necessary to prevent bad outcomes—can sustain symptoms. Rumination and intolerance of uncertainty can maintain a persistent threat model that is resistant to change.
Diagnostic frameworks depend on symptom pattern and duration. Generalized Anxiety Disorder typically involves excessive worry occurring more days than not for at least six months, difficult to control, and accompanied by multiple physical or cognitive symptoms. Panic Disorder requires recurrent unexpected panic attacks with at least one month of concern about additional attacks or maladaptive behavioral change. Specific phobias are marked by intense fear of a particular object or situation, while social anxiety disorder involves fear of social scrutiny or performance situations.
Assessment should include a structured clinical interview, review of symptom onset and course, and screening for comorbidities such as depressive disorders, substance use disorders, and obsessive-compulsive disorder. Medical conditions can mimic or worsen anxiety (e.g., hyperthyroidism, arrhythmias, stimulant use, withdrawal states), so baseline evaluation may be warranted when symptoms are atypical, sudden, or treatment-resistant.
Evidence-based treatment is multimodal. Cognitive Behavioral Therapy (CBT) targets distorted threat appraisals and maladaptive avoidance. Exposure-based strategies allow patients to experience feared cues without catastrophic outcomes, reducing conditioned fear through extinction learning and updated predictions. For panic disorder, interoceptive exposure helps recalibrate fear responses to bodily sensations. Relaxation training and skills such as diaphragmatic breathing can reduce hyperarousal, though they work best when integrated with cognitive and exposure components.
Pharmacotherapy may be appropriate for moderate to severe symptoms, severe functional impairment, or when CBT access is limited. First-line medications often include SSRIs and SNRIs, which modulate serotonergic and noradrenergic signaling involved in threat processing and anxiety regulation. These typically require several weeks for full benefit and should be started low and titrated carefully. Short-term benzodiazepines may reduce acute anxiety but carry risks of sedation, cognitive impairment, dependence, and withdrawal; therefore, they are usually reserved for limited-duration use and careful monitoring.
Sleep and lifestyle interventions can influence symptom severity. Regular sleep schedules, exercise, and limiting caffeine or other stimulants can mitigate physiological arousal. Mindfulness-based approaches and acceptance strategies may reduce fusion with anxious thoughts and improve coping with uncertainty, although protocols vary and should be tailored to patient preferences and diagnosis.
Prognosis is generally favorable with appropriate treatment, but chronicity can occur when avoidance persists, comorbidities are untreated, or medication adherence is inconsistent. Early intervention, psychoeducation, and relapse prevention planning are key. Clinicians should also address suicide risk and severe functional decline when anxiety co-occurs with major depression.
