Behavioral activation is a core CBT-aligned approach that helps patients increase exposure to rewarding or mastery-building activities. Pharmacotherapy is often warranted for moderate to severe depression or when psychotherapy alone is insufficient. These medications modulate synaptic availability of monoamines; clinical improvement generally requires weeks due to downstream neuroplastic changes. Treatment selection also considers comorbid anxiety, insomnia, pain symptoms, and prior medication response.

For treatment-resistant depression, augmentation strategies may include adding another antidepressant class, psychotherapy intensification, or evidence-based somatic options such as electroconvulsive therapy for severe, urgent cases or rapid response. Emerging treatments include ketamine/esketamine in selected settings, with careful monitoring for adverse effects.

Adjunctive lifestyle and supportive interventions matter: regular sleep–wake scheduling, physical activity, social connection, limiting alcohol or other substances, and addressing medical comorbidities. Because depressive symptoms can worsen with untreated medical issues, clinicians should screen for thyroid disease, anemia, and other common contributors. In severe or persistent cases, structured follow-up is essential to monitor symptom trajectory, adherence, and emergent suicidality.
