Evidence-based treatment combines psychotherapy and, when indicated, pharmacotherapy. First-line psychotherapy includes cognitive behavioral therapy (CBT). CBT for GAD targets the cognitive and behavioral maintenance cycles: identifying catastrophic predictions, challenging thought distortions, reducing avoidance, and implementing worry management. Exposure-based techniques may be used to reduce experiential avoidance of uncertainty and bodily sensations. Mindfulness-based approaches can complement CBT by improving attentional control and reducing engagement with worry narratives.

Pharmacotherapy often uses antidepressants with anxiolytic effects. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly prescribed. These medications modulate serotonergic and noradrenergic pathways involved in threat appraisal and arousal regulation. Treatment typically requires several weeks for full benefit, so early follow-up is essential. For acute symptom relief, short-term benzodiazepines may be considered in select cases; however, risks include sedation, cognitive impairment, dependence, and withdrawal, which limits long-term use.

In addition, clinicians may consider adherence to sleep hygiene, regular physical activity, and reduction of excessive caffeine or other stimulants that can amplify physiological arousal. Psychoeducation is crucial: patients learn that worry is a learned threat response rather than a reliable predictor of danger. Relapse prevention emphasizes ongoing skill use, monitoring symptom triggers, and maintaining behavioral activation.

Prognosis is generally favorable with appropriate treatment, but GAD can become persistent if untreated. Comorbidity with major depressive disorder, substance use disorders, and other anxiety disorders is common and affects outcomes. Integrated care—screening for depression, assessing substance use, and addressing medical contributors—improves diagnostic accuracy and treatment response.

Safety considerations include assessment for suicidality and severe functional decline. While GAD itself is not usually associated with immediate medical danger, severe anxiety can co-occur with depression or lead to avoidance that worsens health. Patients should seek medical evaluation when symptoms are new, rapidly worsening, or associated with alarm features such as weight loss, palpitations, or tremor, which may suggest underlying medical pathology.

Overall, GAD is a neurocognitive disorder of threat processing and cognitive control, sustained by worry-related learning and stress-axis sensitization.
