Food insecurity and housing instability are social determinants of health that closely affect cardiometabolic risk, infectious disease vulnerability, mental health, and overall mortality. In health care settings, these conditions often appear indirectly as poor medication adherence, missed appointments, worsening chronic disease, higher emergency department utilization, and depression or anxiety symptoms.

The psychological burden includes uncertainty, perceived loss of control, stigma, and relentless problem-solving. Cognitive load increases as individuals prioritize immediate survival needs, which can impair executive function, concentration, and long-term planning—factors essential for self-management of diabetes, hypertension, asthma, and other chronic conditions.

Behavioral pathways also mediate health impacts. Patients experiencing instability may skip medications, ration insulin, stretch prescriptions, or delay care due to transportation barriers, lack of health literacy support, or competing priorities like food and rent. Missed follow-up visits and inconsistent access to pharmacies lead to fragmented treatment and preventable complications. Emergency care becomes the default safety net, increasing health system strain.

Clinically, screening should be pragmatic and nonjudgmental. Many guidelines recommend assessing food insecurity and housing instability using brief validated tools, such as two-item hunger screening questions and housing stability questions addressing eviction risk, doubling up, or homelessness. Documentation should capture urgency and safety concerns. Clinicians should then link patients to resources: food assistance programs, nutrition support, medically tailored meals, housing navigation, legal aid for eviction prevention, and community health worker support.

Integrating interventions into practice can improve outcomes. For example, coordinating with social services to secure benefits, aligning care plans with realistic food access, and simplifying regimens may improve adherence. Medication choices can be adjusted for patients who cannot reliably take doses with meals. In high-risk cases, clinicians can consider referrals for behavioral health treatment and trauma-informed therapy, while also addressing immediate material needs.

Public health and policy relevance is substantial. While individual clinicians cannot solve macroeconomic constraints, evidence supports that reducing instability and food scarcity improves health trajectories. Housing and food supports may reduce stress physiology, improve diet quality, enhance continuity of care, and lower emergency utilization. For working adults, stability can enable more consistent employment, reliable routines, and better capacity to manage chronic disease.

In summary, food insecurity and housing instability produce measurable biologic and psychosocial harms through HPA-axis dysregulation, inflammation, sleep disruption, nutritional deficiencies, and impaired self-management.
