Avoidance provides short-term relief by reducing anxiety, but it can maintain anxiety by preventing corrective learning. Exposure-based models emphasize that repeated, safe contact with feared cues allows extinction of fear memories and formation of new, non-threatening associations.
Risk factors include a personal or family history of anxiety or mood disorders, traumatic experiences, chronic medical illness, substance use, and certain temperamental traits such as behavioral inhibition. Epidemiologically, anxiety disorders are common and frequently co-occur with depression and other conditions. Medical mimics are important: hyperthyroidism, arrhythmias, medication side effects (e.g., stimulants), and substance withdrawal can present with anxiety-like symptoms. Therefore, assessment should include a careful history, symptom timeline, medication and substance review, and targeted physical evaluation when indicated.
Evidence-based treatments typically combine psychotherapy and, when appropriate, pharmacotherapy. Cognitive behavioral therapy (CBT) is well supported for many anxiety disorders; it includes cognitive restructuring, behavioral experiments, and graded exposure. For GAD, CBT often targets worry processes, problem-solving deficits, and skills for managing somatic tension and sleep. Exposure therapies are central for phobias and panic disorder, using interoceptive exposure for panic by safely re-evoking sensations in a controlled context.
Pharmacologic options vary by diagnosis. First-line medications for many anxiety disorders include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs). These agents can reduce excessive threat appraisal and improve affect regulation, though onset may be gradual. Benzodiazepines may provide short-term symptom relief but carry risks of sedation, cognitive impairment, tolerance, and dependence; they are generally not recommended as long-term monotherapy. For treatment-resistant cases, clinicians may consider other strategies such as buspirone for GAD, specific adjuncts, or referral to specialty care.
Recovery is often measured by reduced symptom frequency and intensity, improved function, and better adaptive coping. Long-term management may involve relapse-prevention planning, continued skill use from therapy, and coordination with general medical care. If anxiety is severe, persistent, or accompanied by suicidal ideation, urgent professional evaluation is warranted.
