
Anxiety disorders are a group of mental health conditions characterized by persistent or excessive fear, worry, and behavioral avoidance that impair functioning. While transient anxiety is normal, clinical anxiety involves dysregulated threat appraisal, heightened arousal, and maladaptive coping that persists despite reassurance. The core symptom domains include cognitive features (excessive worry, rumination, anticipatory threat), emotional features (fear, irritability), somatic arousal (restlessness, muscle tension, sleep disturbance), and behavioral change (avoidance, safety behaviors). Clinically, anxiety exists on a spectrum ranging from anxiety symptoms that fluctuate with stressors to syndromes meeting diagnostic criteria.
Neurobiologically, anxiety disorders reflect altered processing within the amygdala–prefrontal circuitry and dysregulation of threat-related learning. The amygdala, central for salience detection, can become hypersensitive to cues associated with threat. Under such conditions, the medial prefrontal cortex and related executive systems may fail to adequately inhibit amygdala-driven responses, leading to persistent hypervigilance and difficulty returning to baseline after stress. Functional connectivity changes and altered network dynamics have been reported across disorders, emphasizing that anxiety is not simply “overthinking” but a measurable pattern of brain network activity.
At the neurochemical level, multiple systems contribute, including serotonin, norepinephrine, gamma-aminobutyric acid (GABA), and corticotropin-releasing factor (CRF). SSRIs and SNRIs likely exert benefits by modulating serotonergic and noradrenergic signaling, thereby reducing threat sensitivity and improving cognitive control. GABAergic mechanisms are relevant for rapid anxiolysis, explaining why benzodiazepines can reduce acute symptoms through GABA-A receptor–mediated inhibition. However, benzodiazepines can be associated with tolerance, dependence, and cognitive adverse effects, so many guidelines favor limited, time-bounded use or bridging strategies while longer-term therapies take effect.
Diagnostic assessment relies on criteria from standardized classifications. Key elements include duration, intensity, and the extent of impairment. Generalized anxiety disorder (GAD) requires excessive worry across multiple domains on more days than not for at least months, accompanied by symptoms such as difficulty controlling worry, restlessness, fatigue, impaired concentration, irritability, and sleep disturbance. Panic disorder involves recurrent unexpected panic attacks and subsequent worry about additional attacks or behavioral changes. Social anxiety disorder includes fear of social or performance situations due to concerns about negative evaluation. Specific phobias involve circumscribed fear of identifiable stimuli, and separation anxiety disorder reflects fear related to separation from attachment figures.
Differential diagnosis is essential. Anxiety symptoms can reflect medical conditions (e.g., hyperthyroidism, arrhythmias, substance intoxication/withdrawal), medication effects, or other psychiatric disorders such as major depressive disorder, posttraumatic stress disorder, obsessive-compulsive disorder, or adjustment disorders. Comorbidity is common; for example, depressive symptoms frequently co-occur with GAD and panic disorder. Substance use may also maintain anxiety by increasing autonomic arousal or reinforcing avoidance patterns.
Treatment is multimodal and evidence-based. First-line psychotherapy for many anxiety disorders is cognitive behavioral therapy (CBT), which targets the cognitive appraisal and behavioral avoidance loops that maintain anxiety. CBT often includes psychoeducation, interoceptive exposure (for panic), exposure therapy (for phobias and social anxiety), and cognitive restructuring to reduce catastrophic misinterpretation. Through repeated safe exposure and response prevention of safety behaviors, patients learn that feared sensations or outcomes are tolerable and non-fatal, enabling extinction learning and improved inhibitory control.
Pharmacotherapy can be used when symptoms are moderate to severe, when psychotherapy access is limited, or when rapid symptom reduction is required. SSRIs and SNRIs are commonly recommended as first-line medication classes for GAD, social anxiety disorder, and panic disorder. Dose titration is typically gradual to minimize activation-related side effects. Pregabalin may be an option in some settings for GAD, and benzodiazepines may be used short-term for acute distress or during initiation of antidepressants. Beta-blockers may help with peripheral autonomic symptoms such as tremor and tachycardia in performance-related anxiety.
Overall prognosis depends on early recognition, treatment adherence, and addressing maintaining factors such as avoidance, poor sleep, substance use, and ongoing stress. CBT-informed relapse prevention helps patients sustain gains by continuing skills practice, managing setbacks, and maintaining exposure schedules.
Nonpharmacologic supports—sleep hygiene, regular physical activity, mindfulness-based strategies, and reduction of caffeine or stimulants—can complement formal treatment. However, these should not replace evidence-based psychotherapy or medication when diagnostic criteria are met.
If anxiety symptoms are persistent, escalating, or impairing, clinical evaluation is warranted to establish the correct diagnosis and to rule out medical causes. With appropriate assessment and evidence-based interventions, many individuals achieve meaningful and durable symptom reduction.
