Anxiety disorders are a group of conditions characterized by excessive fear, worry, and hyperarousal that are disproportionate to circumstances and persist over time, leading to functional impairment. The core clinical feature is not merely occasional nervousness but sustained or recurrent symptoms that create distress and/or interfere with work, relationships, sleep, and physical health. Common anxiety diagnoses include generalized anxiety disorder (GAD), panic disorder, social anxiety disorder, specific phobias, and agoraphobia, each with distinctive symptom patterns and triggers.

From a mechanistic standpoint, anxiety involves coordinated dysregulation across neural circuits responsible for threat detection, salience processing, and defensive responses. The amygdala and related limbic structures contribute to heightened threat appraisal, while prefrontal regulatory networks (e.g., medial and dorsolateral prefrontal cortex) may inadequately suppress threat signals during stress. Functional neuroimaging studies commonly show altered connectivity between limbic regions and cognitive control areas, which can manifest as exaggerated reactivity to ambiguous or minor cues.

At the neurotransmitter and biological systems level, anxiety is associated with perturbations in gamma-aminobutyric acid (GABA), serotonergic signaling, noradrenergic tone, and stress-hormone regulation. In many patients, chronic worry and hypervigilance coincide with autonomic nervous system activation: increased sympathetic output can drive palpitations, tremor, sweating, gastrointestinal upset, and sleep disruption. The hypothalamic-pituitary-adrenal (HPA) axis may also show altered feedback, promoting a cycle of stress amplification and impaired recovery.

Cognitive-behavioral models emphasize that anxiety is sustained by maladaptive appraisal and avoidance. For GAD, repetitive negative thinking, excessive uncertainty tolerance intolerance, and biased threat prediction create a worry loop that becomes reinforcing through short-term relief (e.g., reassurance-seeking) and long-term reinforcement of perceived danger. For panic disorder, catastrophic misinterpretation of bodily sensations (“this must mean something dangerous”) can trigger escalating autonomic arousal, forming a self-perpetuating panic cycle. Social anxiety disorder is maintained by fear of negative evaluation and safety behaviors that prevent disconfirming experiences.

Clinically, diagnosis relies on DSM-5-TR criteria and careful differential assessment. Key criteria include symptom duration, intensity, and the degree to which symptoms are not better explained by another mental disorder, medical condition, or substance/medication effect. Somatic symptoms—such as restlessness, muscle tension, fatigue, concentration difficulties, irritability, and sleep disturbance—are particularly relevant in GAD. Panic disorder is characterized by recurrent unexpected panic attacks followed by persistent concern about additional attacks or maladaptive behavior changes. Social anxiety requires fear of scrutiny in social performance or interaction contexts.

A crucial component is medical and substance rule-out, because anxiety-like symptoms can result from hyperthyroidism, pheochromocytoma, arrhythmias, medication side effects (e.g., stimulants), withdrawal states, and stimulant or substance use. Clinicians also screen for comorbidities: major depressive disorder, obsessive-compulsive disorder, posttraumatic stress disorder, and substance use disorders are frequently intertwined with anxiety and may shape treatment selection.

Evidence-based treatment is typically multimodal, targeting both symptom generators and maintenance mechanisms. Psychotherapy is foundational. Cognitive behavioral therapy (CBT) helps patients identify cognitive distortions, reduce avoidance, and practice graded exposure to feared cues. Exposure therapy is especially effective for specific phobias, social anxiety disorder, and panic-related avoidance, because repeated nonreinforced approach diminishes conditioned fear responses. For GAD, CBT targets worry processes, intolerance of uncertainty, and problem-solving skills.

Pharmacotherapy is often used when symptoms are severe, chronic, or refractory to psychotherapy alone. First-line medication options commonly include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) due to their modulatory effects on serotonergic and noradrenergic systems. These agents may take several weeks for full benefit, reflecting neuroadaptive changes rather than immediate anxiolysis. In select cases, buspirone (a partial agonist at 5-HT1A receptors) can be used for GAD.

Benzodiazepines (e.g., clonazepam, lorazepam) can provide short-term relief by enhancing GABA-A mediated inhibition, but their use requires caution due to sedation, impaired coordination, tolerance, dependence, and withdrawal risk. They are often reserved for brief periods or acute exacerbations while longer-term therapies take effect. Clinicians also consider sleep optimization and management of comorbid depression or insomnia.

Lifestyle and supportive interventions can complement evidence-based care. Regular aerobic exercise, consistent sleep schedules, reduction of caffeine and stimulants, and mindfulness-based strategies may reduce baseline arousal and improve coping. However, these measures are adjunctive and should not replace psychotherapy or medication when clinically indicated.

Prognosis varies by diagnosis, comorbidity, and adherence to treatment. Many patients experience substantial improvement with structured CBT and/or guideline-concordant pharmacotherapy. Early intervention is associated with better functional outcomes and reduced risk of chronicity.
