Pharmacotherapy can be used when symptoms are moderate to severe, function is significantly impaired, or rapid symptom reduction is needed.

Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly recommended because of robust efficacy and tolerability profiles. Treatment typically requires several weeks to achieve full benefit; early activation-related side effects should be monitored. For specific cases, short-term benzodiazepines may be considered for acute relief, but risks include sedation, impaired coordination, dependence, and withdrawal; thus they are generally not favored as long-term solutions. Additional options may include buspirone for GAD, and specialized agents or augmentation strategies under psychiatric guidance when partial response occurs.

Management also includes addressing sleep, reducing caffeine or stimulant exposure, treating comorbid depression or substance use, and implementing stress reduction strategies such as mindfulness-based approaches where appropriate. Lifestyle interventions—regular aerobic exercise, structured routines, and social support—can improve baseline arousal regulation and enhance treatment response.

Prognosis varies by disorder subtype, comorbidity, and duration of untreated illness. Early, evidence-based interventions improve outcomes, while chronic avoidance and untreated comorbidities increase persistence and disability. A careful, patient-centered diagnostic formulation—integrating neurobiological vulnerability, cognitive mechanisms, and learned safety behaviors—supports selection of the most effective therapy and reduces relapse risk.
