Physiologically, the worry process increases sympathetic arousal, contributing to muscle tension, restless behavior, and sleep disruption. These symptoms can reinforce the cognitive model: perceived bodily sensations become additional cues of danger, perpetuating the cycle.

CBT for GAD often employs techniques such as cognitive restructuring, problem-solving training, stimulus control for sleep, and gradual exposure to avoided feared situations. Another effective approach is intolerance-of-uncertainty interventions, which systematically reduce reliance on reassurance seeking and foster adaptive acceptance of uncertainty. Mindfulness-based cognitive approaches may help by reducing attentional fixation on worry.

Pharmacotherapy is also widely used, particularly when symptoms are severe, persistent, or when psychotherapy access is limited. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly recommended as first-line medications. These agents modulate serotonergic and noradrenergic signaling, which can reduce anxiety intensity and improve cognitive control over worry. Treatment typically requires several weeks for full effect, and clinicians monitor side effects such as gastrointestinal upset, sleep changes, sexual dysfunction, and initial activation. Benzodiazepines can reduce acute anxiety symptoms but are generally not preferred for long-term management due to risks of tolerance, dependence, withdrawal, and cognitive impairment.

A key aspect of care is differential diagnosis and risk assessment. Clinicians must rule out medical causes of anxiety (e.g., hyperthyroidism, medication-induced anxiety, substance-related syndromes) and evaluate for suicide risk or severe comorbid depression. They also consider bipolar spectrum disorders because antidepressant monotherapy in bipolar disorder can precipitate mania or hypomania. Sleep evaluation is important; insomnia can both worsen and be worsened by anxiety, so integrated sleep hygiene and behavioral interventions are often beneficial.

Prognosis is variable but often favorable with sustained treatment engagement. Symptom trajectories can improve with CBT and pharmacotherapy, especially when patients learn to interrupt avoidance and reduce worry-based safety behaviors. Relapse prevention includes maintaining skills from therapy, managing life stressors, and coordinating care for comorbid conditions.

Its mechanisms reflect interacting cognitive biases, neurocircuit dysregulation of threat and control systems, and stress-related physiology.
