
Anxiety disorders are a group of conditions characterized by excessive fear, worry, or nervous system arousal that is disproportionate to circumstances and causes clinically significant distress or impairment. They include generalized anxiety disorder (GAD), panic disorder, social anxiety disorder (social phobia), specific phobias, and anxiety symptoms associated with other disorders. At the mechanistic level, anxiety reflects dysregulation within cortico-limbic circuits—particularly the amygdala, hippocampus, anterior cingulate cortex, and prefrontal regulatory networks—along with alterations in neurochemical signaling that modulate threat detection, salience attribution, and stress reactivity.

The worry is accompanied by symptoms such as restlessness, fatigue, impaired concentration, irritability, muscle tension, and sleep disturbance. The pathophysiology involves heightened threat expectation, intolerance of uncertainty, and cognitive attentional bias toward potential negative outcomes. Stress response systems are also implicated: both dysregulated hypothalamic–pituitary–adrenal (HPA) axis activity and altered autonomic arousal can contribute to sustained somatic and cognitive symptoms. Neurobiologically, altered serotonergic and noradrenergic signaling is commonly discussed, with downstream effects on prefrontal control and amygdala reactivity.
Panic disorder presents with recurrent unexpected panic attacks—abrupt episodes of intense fear accompanied by palpitations, sweating, trembling, dyspnea, chest discomfort, nausea, dizziness, paresthesias, or fear of losing control or dying. A key maintaining mechanism is catastrophic misinterpretation of benign bodily sensations, producing anticipatory anxiety and conditioned fear responses. Interoceptive sensitivity (heightened awareness of internal cues) and impaired safety learning play central roles. This is why treatment often targets both catastrophic cognitions and exposure to bodily sensations.
Social anxiety disorder involves marked fear of social situations in which embarrassment or scrutiny may occur. The clinical pattern reflects hypervigilance for negative evaluation and safety behaviors (e.g., hiding, rehearsing, avoiding eye contact) that prevent corrective learning. Over time, these behaviors maintain anxiety by reducing opportunities to disconfirm feared outcomes. Specific phobias are driven by circumscribed fear triggers and are maintained by avoidance; extinction and exposure learning can be highly effective.
Diagnosis is clinical and requires careful differentiation from medical causes (e.g., hyperthyroidism, arrhythmias, medication or substance effects, stimulant use), substance-induced anxiety, and other mental disorders such as major depressive disorder or obsessive-compulsive disorder (OCD). Standard criteria emphasize severity, duration, and functional impact, and clinicians assess comorbidity, including depression, substance misuse, and sleep disorders, which are common in anxiety syndromes.
Evidence-based treatment is multimodal. First-line psychotherapy for many anxiety disorders includes cognitive behavioral therapy (CBT), which uses cognitive restructuring, behavioral experiments, and graded exposure to reduce fear and avoidance. For GAD, CBT often focuses on worry control, intolerance of uncertainty, problem-solving skills, and reduction of reassurance seeking. Exposure therapy is central to panic disorder and phobic disorders: it allows extinction learning by repeatedly confronting feared sensations or situations without catastrophic consequences. Acceptance-based approaches may also help by reducing experiential avoidance.
Pharmacotherapy is guided by symptom profile and comorbidities. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used due to evidence for reducing core anxiety symptoms and preventing relapse. For severe or acute symptoms, some guidelines allow short-term use of benzodiazepines with caution because of risks including sedation, cognitive impairment, dependence, and withdrawal. Medication initiation typically requires patient education about delayed onset and the need for adherence. In panic disorder and social anxiety disorder, gradual titration is often used to reduce early transient worsening.
Adjunctive strategies include sleep optimization, regular aerobic exercise, mindfulness-based interventions (as complements to CBT), stress management, and reducing caffeine or other stimulants that can exacerbate autonomic arousal. In selected cases, neurobiological or neuromodulation approaches may be considered, but they are not first-line for most patients.
Prognosis varies by disorder subtype, duration of illness, and comorbidity. Early identification and consistent treatment generally improve outcomes. Longitudinal studies suggest that combining psychotherapy with pharmacotherapy can be beneficial when symptoms are severe or comorbid depression is present. Relapse prevention plans—reinforcing coping skills, exposure maintenance, and addressing life stressors—are key.
In clinical practice, the most effective care addresses not only symptoms but also the cognitive and behavioral mechanisms that sustain anxiety: threat overestimation, avoidance, safety behaviors, and intolerance of uncertainty. With structured assessment, evidence-based therapy, and appropriate medication when needed, many individuals achieve substantial symptom reduction and functional recovery.
