Anxiety Disorders: Neurobiology, DSM-5 Criteria, Differential Diagnosis, and Evidence-Based Treatment Strategies

Anxiety disorders are a group of mental disorders characterized by excessive fear, worry, or nervous system arousal that is disproportionate to circumstances and persists over time. While anxiety can be adaptive in signaling threat, pathological anxiety involves maladaptive threat appraisal, heightened physiological reactivity, and impaired emotion regulation. In clinical practice, anxiety disorders span generalized anxiety disorder (GAD), panic disorder, social anxiety disorder (SAD), specific phobias, and anxiety related to trauma and stressor exposure. The unifying feature is persistent or recurrent anxiety that causes clinically significant distress or functional impairment.

Neurobiologically, anxiety is linked to dysregulation across cortico-limbic circuits. The amygdala plays a central role in threat detection and salience assignment. Hyperactivity within amygdala-centered networks, combined with altered prefrontal regulation, can reduce top-down control over fearful or worrisome thoughts. The bed nucleus of the stria terminalis and other extended amygdala structures contribute to sustained anxiety and stress reactivity. Neurotransmitter systems implicated include serotonin, norepinephrine, gamma-aminobutyric acid (GABA), and glutamate. GABAergic inhibitory dysfunction may permit exaggerated arousal, while serotonergic and noradrenergic alterations affect worry, vigilance, and sleep. Functional imaging studies commonly show abnormal connectivity between threat-related regions and regulatory networks, consistent with persistent perception of threat even when cues are ambiguous.

Cognitively, anxiety disorders are maintained by attentional bias toward threat, interpretation bias, and negative metacognitive beliefs. In GAD, worry functions as a maladaptive cognitive control strategy: repetitive mental problem-solving intended to reduce uncertainty can paradoxically increase distress and strengthen threat-related neural representations. Intolerance of uncertainty is a robust maintaining factor. In panic disorder, misinterpretation of benign bodily sensations (e.g., palpitations) as dangerous can create a feedback loop between interoceptive cues, catastrophic thinking, and panic symptoms. In SAD, fear centers on social evaluation and scrutiny, with performance-related safety behaviors that prevent corrective learning, thereby sustaining avoidance.

DSM-5 diagnostic frameworks require symptom duration, severity, and impairment criteria. GAD typically involves excessive anxiety and worry occurring more days than not for at least six months, with difficulty controlling the worry and additional symptoms such as restlessness, being easily fatigued, difficulty concentrating, irritability, muscle tension, or sleep disturbance. Panic disorder involves recurrent unexpected panic attacks and persistent concern about additional attacks or maladaptive behavior related to the attacks. Social anxiety disorder involves marked fear of social or performance situations due to possible negative evaluation, with avoidance or enduring distress.

Differential diagnosis is essential. Anxiety symptoms may arise from depressive disorders, trauma-related disorders, obsessive-compulsive and related disorders, and medical causes such as hyperthyroidism, cardiac arrhythmias, pulmonary disease, stimulant intoxication, or withdrawal states. Substance-induced anxiety must be considered when onset correlates with medication changes, drug use (including caffeine or cannabis), or abrupt discontinuation of sedatives. Sleep disorders, chronic pain, and neurologic conditions can also amplify arousal and mimic anxiety. A careful history, mental status examination, and targeted laboratory evaluation are often warranted.

Treatment is multimodal and evidence-based. First-line psychotherapy includes cognitive-behavioral therapy (CBT), which targets cognitive distortions, attentional biases, and avoidance. CBT often incorporates cognitive restructuring, exposure strategies, behavioral experiments, and relapse prevention. For phobias and SAD, graded exposure with response prevention of safety behaviors supports extinction learning and corrective interpretation. Pharmacotherapy frequently includes selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modulate serotonergic and noradrenergic signaling over weeks and reduce core worry and fear responses. Evidence supports the use of buspirone for GAD in some populations. Short-term benzodiazepines can reduce acute symptoms but carry risks of sedation, falls, dependence, tolerance, and cognitive impairment; thus they are generally reserved for limited durations and carefully selected patients.

Adjunctive strategies may include mindfulness-based approaches, sleep hygiene, reduction of stimulants, and stress management. Lifestyle interventions that improve circadian stability, reduce caffeine and alcohol, and increase physical activity can lower baseline arousal. In treatment-resistant cases, augmentation strategies may include careful consideration of dose optimization, switching within the same class, or evidence-informed add-ons under psychiatric supervision.

Safety considerations include monitoring for worsening anxiety at treatment initiation for some patients, assessment for comorbid suicidality in depressive co-morbidity, and management of medical causes. Effective care also includes psychoeducation: explaining that anxiety is a false alarm mechanism driven by learning and threat circuitry, and that symptom reduction reflects recalibration of threat appraisal rather than elimination of stressors.

In summary, anxiety disorders reflect neurobiological threat-salience dysregulation, cognitive biases that maintain worry and fear, and behavioral patterns that prevent corrective learning. Diagnosis requires persistent symptoms and rule-out of medical and substance causes. Treatment integrates CBT-based mechanisms with first-line SSRIs/SNRIs, and when appropriate, targeted short-term anxiolytics. With appropriate intervention, many patients achieve substantial symptom remission and improved functioning.
