
Anxiety disorders are a group of conditions marked by excessive fear, worry, and physiological hyperarousal that impair functioning and persist beyond normative stress responses. Clinically, they include generalized anxiety disorder (GAD), panic disorder, social anxiety disorder, specific phobias, agoraphobia, and separation anxiety (with age-dependent expression). Although anxiety can be adaptive, pathological anxiety is characterized by disproportionate threat appraisal, reduced control over symptoms, and sustained activation of threat-related neural circuits.
From a neurobiological standpoint, anxiety involves coordinated dysfunction across the amygdala, prefrontal cortex, hippocampus, and brainstem pathways that regulate arousal. The amygdala rapidly tags potential threat cues, while the medial and lateral prefrontal cortex normally modulates and reappraises those cues to prevent overreaction. In many patients, impaired top-down control and heightened salience processing contribute to persistent worry or panic. The locus coeruleus–norepinephrine system is often implicated in the physiological symptoms of anxiety—tachycardia, trembling, sweating, and hypervigilance—reflecting an exaggerated autonomic readiness state. Cognitively, anxiety disorders are maintained by maladaptive threat beliefs and attentional and interpretive biases. In GAD, repetitive worry functions as an attempted cognitive control strategy: individuals may believe worry prevents bad outcomes, even when it fails to reduce anxiety and worsens distress. This pattern aligns with intolerance-of-uncertainty frameworks, where uncertainty itself becomes threatening. Worry reduces immediate anxiety through negative reinforcement but is maintained by delayed catastrophic anticipation. In panic disorder, catastrophic misinterpretation of bodily sensations (e.g., palpitations interpreted as danger) triggers fear, which then amplifies autonomic arousal, creating a self-reinforcing cycle. In social anxiety disorder, negative self-evaluation and fear of scrutiny drive avoidance; attention often shifts toward perceived flaws, and post-event rumination sustains symptom chronicity.
Behaviorally, avoidance and safety behaviors prevent corrective learning. Avoidance removes opportunities to disconfirm feared predictions, while safety behaviors (e.g., rehearsing, masking anxiety, using alcohol to cope) can reduce immediate fear but block habituation. Over time, the feared situation generalizes, increasing functional impairment. For example, in panic disorder with agoraphobic patterns, repeated avoidance of environments associated with past attacks can entrench anticipatory anxiety.
Clinically, diagnosis depends on symptom clusters, duration, severity, and rule-out of substance-induced and medical causes. Common symptoms include persistent worry, restlessness, fatigue, irritability, muscle tension, sleep disturbance, and impaired concentration in GAD; recurrent unexpected panic attacks, worry about additional attacks, or behavioral change in panic disorder; and fear of embarrassment, blushing, or negative evaluation in social anxiety disorder. Differential diagnosis is essential because thyroid disease, arrhythmias, pheochromocytoma, substance intoxication/withdrawal, and certain medications can mimic anxiety. Assessment should also address comorbid depression, obsessive-compulsive symptoms, trauma-related conditions, and substance use.
Evidence-based treatment typically combines psychotherapy and pharmacotherapy tailored to disorder subtype and patient preferences. First-line psychotherapy for many anxiety disorders includes cognitive behavioral therapy (CBT), which targets maladaptive thoughts, attentional biases, and avoidance. For GAD, CBT often incorporates cognitive restructuring, worry exposure, and problem-solving, while also teaching metacognitive and mindfulness-based strategies to reduce time spent in worry. For panic disorder, CBT includes interoceptive exposure and cognitive restructuring of panic misinterpretations to break the panic cycle. For phobias and agoraphobic avoidance, exposure-based interventions are central; graded, repeated exposure promotes extinction learning and increases perceived coping ability.
Pharmacologic options may include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modify serotonergic and noradrenergic pathways implicated in threat processing and arousal. These agents often take weeks to achieve full benefit and may initially increase anxiety in some patients, necessitating careful titration and psychoeducation. Short-term benzodiazepines can reduce acute symptoms through GABA-A receptor modulation, but their use is typically limited due to risks of sedation, falls, cognitive impairment, and dependence. Buspirone may be considered for some patients with GAD because of its partial agonist activity at serotonin 5-HT1A receptors.
Prognosis varies but is generally improved with early, evidence-based intervention. Risk factors for chronicity include persistent avoidance, comorbid depression, traumatic history, ongoing substance misuse, and poor adherence. Functional recovery is best supported by structured follow-up, skill reinforcement, and relapse prevention planning. Lifestyle factors—regular sleep, exercise, and reducing stimulant use—can support symptom control but are not substitutes for targeted therapy.
