Although anxiety can be adaptive, anxiety disorders involve maladaptive patterns of threat appraisal, heightened autonomic reactivity, and/or difficulty disengaging from perceived risk. Catastrophic misinterpretation of bodily sensations (interoceptive threat) is central to panic disorder: symptoms such as dizziness or palpitations are misread as dangerous, triggering a feedback loop of fear, further symptom escalation, and panic attacks. In social anxiety disorder, negative beliefs about social evaluation and heightened self-focused attention contribute to avoidance and impairment.

Treatment is evidence-based and typically multimodal. Pharmacotherapy may be indicated when symptoms are severe, persistent, or impairing, or when psychotherapy alone is insufficient. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) are commonly used as first-line medications for several anxiety disorders, with gradual onset over weeks. Benzodiazepines can provide short-term symptomatic relief but carry risks including sedation, tolerance, dependence, and withdrawal; they are generally reserved for acute episodes or bridging strategies under careful supervision. Other options—depending on the disorder—include buspirone for GAD, beta-blockers for performance-related symptoms in some cases, and trauma-focused interventions for PTSD. Medication selection must account for comorbid depression, sleep disorders, substance use risk, pregnancy/lactation, and drug interactions.
Anxiety disorders also require attention to lifestyle and medical contributors. Sleep regularity, limiting caffeine and other stimulants, reducing alcohol use, and promoting graded physical activity can decrease physiological arousal. Stress management strategies (e.g., mindfulness-based approaches, diaphragmatic breathing) may reduce reactivity, though they typically work best as adjuncts to disorder-specific therapy.
Prognosis is influenced by early intervention, treatment adherence, comorbidity management, and avoidance reduction. With appropriate treatment, many individuals experience substantial symptom improvement, though relapse prevention and ongoing coping skills remain important.
