From a biopsychosocial perspective, anxiety arises when perceived threats—whether internal (uncertainty, intrusive thoughts) or external (environmental stressors)—outweigh the perceived ability to cope. Poor air quality has been associated with higher rates of anxiety-like symptoms in observational studies, potentially via systemic inflammation, oxidative stress, and neural effects of particulate matter. Sleep quality is another critical pathway; inadequate sleep amplifies amygdala reactivity and impairs prefrontal regulation, making worry more persistent. Diet and gut-brain signaling may also contribute: inflammatory diets, micronutrient deficiencies, and dysbiosis can affect autonomic tone and stress reactivity, while adequate hydration supports cognitive performance and may reduce fatigue-related triggers for worry.
Cognitive models explain maintenance through thought processes such as intolerance of uncertainty, catastrophic misinterpretation of bodily sensations, and attentional bias to threat cues. For example, normal variability in heartbeat can be misread as danger, creating a feedback loop: arousal increases, threat interpretation intensifies, and arousal rises again. Behavioral avoidance—staying away from situations that provoke worry—reduces short-term distress but prevents disconfirmation of feared outcomes, sustaining anxiety over time.
Evidence-based treatment typically combines psychotherapy, and when indicated, pharmacotherapy. Cognitive behavioral therapy (CBT) targets maladaptive beliefs, teaches cognitive restructuring, and includes exposure techniques to reduce fear-avoidance patterns. Mindfulness-based approaches and acceptance-oriented therapies can reduce experiential avoidance and improve emotion regulation. Pharmacologically, first-line options for chronic anxiety commonly include selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs), which modulate threat and arousal networks over several weeks. Short-term symptom relief may involve carefully selected benzodiazepines, though these carry risks of sedation, dependence, and impaired cognition; they are usually limited in duration and context. Buspirone, pregabalin (where appropriate), and other agents may be considered depending on diagnosis and comorbidities.
Prevention and self-management focus on stabilizing physiological arousal and reducing cognitive reinforcement. Regular physical activity, consistent sleep scheduling, breathing or relaxation training, and structured problem-solving can lower baseline sympathetic activation. Reducing exposure to known stressors, improving environmental conditions where feasible, and building supportive relationships can enhance perceived control and safety—core determinants of anxiety trajectories. Given the interplay between anxiety and physical health, persistent symptoms warrant assessment for comorbid depression, thyroid disease, medication effects (e.g., stimulants), substance use, and medical conditions that mimic anxiety.
If anxiety causes significant impairment—such as inability to work, recurrent panic, severe insomnia, or escalating somatic symptoms—clinical evaluation is recommended. Early, accurate diagnosis improves outcomes, because targeted interventions can interrupt the neurocognitive feedback loop sustaining worry and stress reactivity.
